A catheter device and methods useable to extract material from a body conduit, e.g., a blood vessel, are provided. The catheter comprises a flexible catheter advanceable into the body conduit, an opening in a wall of the catheter in fluid communication with a material collection chamber, and a controllably arcuate segment near the distal tip of the catheter and the opening. The catheter may include a sliding member, located within a lumen of the catheter, used to move the material entering the catheter through the arcuate segment opening, into the material collection chamber and away from said opening. The catheter may include a single mechanism utilized to both generate a vacuum to cause material to enter the catheter at the arcuate segment opening and also cause the sliding member to travel, inside the catheter, moving material away from the opening and into the material collection chamber.

Methods and devices for implanting pacing electrodes or other apparatus, or for delivering substances, to the heart of other tissues within the body. A guided tissue penetrating catheter is inserted into a body lumen (e.g., blood vessel) or into a body cavity or space (e.g., the pericardial space) and a penetrator is advanced from the catheter to a target location. In some embodiments, a substance or an apparatus (such as an electrode) may be delivered through a lumen in the penetrator. In other embodiments, a guidewire may be advanced through the penetrator, the penetrating catheter may then be removed and an apparatus (e.g., electrode) may then be advanced over that guidewire. Also disclosed are various implantable electrodes and electrode anchoring apparatus.

The systems and devices have a reduced number of discrete components that allow placement through small incisions and tubes. More particularly, the present invention is directed to systems and methods of treating the spine, which eliminate pain and enable spinal motion, which effectively mimics that of a normally functioning spine. Methods are also provided for stabilizing the spine and for implanting the subject systems.

Data from the joint movement device is transmitted to the processing unit for determining a value of a configuration parameter of the system. Also disclosed is a joint movement device including a flexible structure for applying force to one or more patient joints, and controlled cables that produce the forces required.

A tissue penetrating catheter that is usable to advance a tissue penetrator from within a blood vessel, through the wall of the blood vessel to a target location. The catheter includes at least one stabilizing device thereon for stabilizing catheter prior to advancing the tissue penetrator. The tissue penetrator may extend through a lumen in the body of the catheter and project transversely through an exit port. The stabilizing device may be located closely adjacent to the exit port, or may surround the exit port. The stabilizing device may be one or more balloons, or other mechanical structure that is expandable into contact with the inner luminal wall of the blood vessel. Desirably, the exit port is forced into contact with the blood vessel wall to shorten the distance that the tissue penetrator projects from the catheter body to the target location. The catheter is particular useful for forming blood flow tracts between blood vessels, in particular in coronary revascularization procedures. Methods of utilizing such a catheter to bypass an arterial obstruction is also disclosed.

The invention provides a wire guided aperture creating apparatus, an over-the-wire kit, and a method for creating a hole in adjacent portions of first and second anatomical conduits during a surgical procedure. The apparatus includes an elongated element having a guidewire lumen and an aperture creating assembly disposed near the distal end of the elongated element. In a further embodiment the device may include first and second elongated elements coupled via a coupler and an aperture creating assembly disposed between the elements. At least one of the elements includes a passage-defining element permitting passage therethrough of a first guidewire. In addition to the devices identified above, the kit includes a first guidewire for insertion into a lumen of the first vessel to an initial cutpoint and then through adjacent portions of sidewalls of the first and second vessels into a lumen of the second vessel. The method involves inserting a first guidewire into a lumen of a first vessel to an initial cutpoint and then though adjacent portions of sidewalls of the first vessel and a second vessel into a lumen of the second vessel. The method further involves providing a wire guided aperture creating apparatus as identified above, and inserting the device over the first guidewire such that the passage-defining element passes over the first guidewire.

A changeable length of the shape memory element decreasing from an uncharged length to a charged length resets the return element.

Devices and methods for achieving directed placement of guidewires or other flexible rails over which other catheters or other devices may be introduced to a targeted location. A guidewire directing device includes a lumen extending between a proximal end and a distal peripheral opening for directing a guidewire laterally from the device, and may have a deflecting member with a predetermined deflection angle in the lumen adjacent the peripheral opening. A snaring device is also provided that includes one or more lumens through which a snare or other grasping member is directed for releasably capturing or coupling to a guidewire. The snaring and target guidewire devices may include cooperating end effectors for releasably coupling or securing the devices together. The devices may be used for directing, snaring and/or manipulating one or more guidewires between two blood vessels connected by an interstitial channel, for example, to “floss” a guidewire through the coronary arterial and venous systems between two percutaneous entry sites and/or to bypass a lesion in a vessel using proximal and distal interstitial channels communicating with an adjacent vessel.
