A surgical treatment for restoring proper anatomical spacing and alignment to vertebral bones including: determining an angular misalignment associated with adjacent vertebral bones; sequentially inserting and removing a series of progressively wider spacer elements into the corresponding intervertebral space between the adjacent vertebral bones until the proper anatomical spacing between the adjacent vertebral bones is restored; for each intervertebral space, inserting a tapered porous spacer element into the intervertebral space between the corresponding adjacent vertebral bones; rotating the tapered porous spacer element such that the rotational orientation of the tapered porous spacer element introduces the appropriate counter offset to the intervertebral space of the previously misaligned scoliotic vertebral bones, thereby restoring the proper anatomical alignment of the vertebral bones; and stabilizing the adjacent vertebral bones to permit infused growth of bone into the tapered porous spacer element.

A polyaxial locking cervical screw and plate assembly for immobilization of cervical bones, via fixation to surfaces thereof, including a plate, having a pair of elongated tapered holes in the top and bottom thereof, into which holes elongated coupling elements and screws may be inserted. Each coupling element has an equivalent taper which matches the taper of the holes in the plate, and an interior semi-spherical curvate surface in which the curvate head of the screw may be polyaxially mounted. The coupling elements are initially disposed in the holes in the plate such that they may slide axially therein. The bone screws are inserted through the respective coupling elements until the heads thereof enter the curvate volumes thereof. Once the head is fully seated in the coupling element, advancement of the screw causes the coupling element to crush lock to the plate and to the head of the screw.

A modular pedicle screw and rod implant assembly includes a shant screw, a rod holding element, a locking member, a rod, and a nut. The shant screw has a lower threaded shaft portion for insertion into the pedicle and a top shaft portion which is unthreaded, a portion of which is tapered. The locking member has a threaded upper portion and a smoothly tapered lower portion. It further includes an axial bore which is similarly tapered at the bottom for seating on the upper portion of the shant screw. This tapered bottom portion is slotted so that it may be expanded or compressed in accordance with a radial force applied thereto. The rod holding element includes two through holes; one for receiving the rod, and the other, transverse to the rod receiving hole, which is tapered. The two holes are overlapping so that when the rod is seated in the first hole and the tapered portion of the locking member is in the second hole the surfaces of the rod and the locking member are in contact. The upper portion of the locking member is threaded, and receives the nut. Tightening of the nut causes the rod and locking member to compress against one another, and for the locking member to compress against the top of the shaft screw, thus locking the assembly together securely.

The present invention is a selectively expandable bone fixation screw-sleeve device which provides substantially greater holding strength in human bone than other bone screw devices. The sleeve element of this invention includes several embodiments, each having at least one corresponding screw element for selectively expanding a portion of the sleeve once it has been inserted into the bone. More specifically, in several embodiments, the sleeve includes a series of vertical slots in the end, or ends, of the cylindrical body of the sleeve. The slots define, therebetween, sections which are contiguous with the remainder of the cylindrical body, and which may rotate radially outward. In these embodiments, the sections are designed having a varying thickness, for example a single or double wedge profile, such that the insertion of the screw element into the sleeve deforms the shape of the sleeve by causing the sections to flair outward. In a different embodiment, the slots, and sections, are designed into the axially intermediate central portion of the cylindrical sleeve, and therefore flair outwardly therefrom upon insertion of a screw within the sleeve. As this invention is intended to provide imporved fixation support for orthopedic implantation assemblies, each embodiment may further include means coupled or integral with the sleeve or screw for coupling an orthopedic implant to it.

A unitary locking cap mechanism, which is used with pedicle screws and other orthopaedic implant devices which couple bones to rods, includes rim and post portions which are rotationally freely coupled to one another to permit independent motion relative to one another. The rim element has a cylindrical body with an open end and a closed end; the closed end alternatively having a concentric hole defined by an inwardly directed flange. In one embodiment, the flange includes a series of slots, dividing into a plurality of wedge-shaped pie sections. In another embodiment, the sidewall of the cylindrical portion of the rim includes an annular recess. The post is a cylindrical shaft having surface elements at one end which provide for mutual engagement with the rim. These surface elements may be deflecting arms which seat into the annular recess of the rim, or an annular recess in the shaft into which the wedge-shaped pie sections of the flange portion may be resiliently snapped into. In an alternative embodiment, the shaft and the rim are coupled together through the concentric hole by a retaining ring which is placed around the shaft, in an annular recess therein, after the insertion of the shaft through the hole. Each embodiment are alternatively designed such that the post or the inner surface of the rim portions are threaded so that it can mate with a threading on the inner or outer upwardly extending, U-shaped channel forming, members of the heads of the pedicle screws.

An orthopaedic cage device having a rectangular cross-section and an intervertebral space shape conforming structure is formed of two opposing shell elements being hinged at one end to form an interior volume therebetween. The surface of the interior volume is tapered in the axial direction. A threaded shaft is axially disposed in the interior volume and is held in place by a retaining ring slideably mounted at the non-hinged axial end of the device. A nut, being of substantially the same dimension as the maximum cross-section of the interior volume in its initial disposition, is disposed on the threaded shaft such that rotation of the shaft causes the nut to translate axially within the interior volume. This translation causes the nut to engage the tapered surface of the interior volume, which in turn causes the non-hinged end of the device to spread such that the device conforms to the natural space between the vertebral bones and provides for the proper curvature of the spine.
