A catheter apparatus for removing an obstruction within a body lumen includes an elongate tubular shaft defining a lumen and a flexible membrane that fluidly seals the distal end of the tubular shaft. At least one cutter member is attached to and distally extends from the flexible membrane. An actuating mechanism is operatively connected to a proximal end of the tubular shaft. The actuating mechanism displaces a fluid disposed within the lumen of the tubular shaft in such a manner that the fluid oscillates the flexible membrane and the cutter member attached thereto. Accordingly, the catheter apparatus uses pulsatile fluid flow through the tubular shaft to transmit energy from the driving mechanical at the proximal end of the catheter apparatus to the flexible membrane at the distal end of the catheter apparatus. The transmitted energy causes the cutting member to oscillate and break up a target blood clot.

The devices of the present disclosure are rheolytic thrombectomy catheters with a self-inflating distal balloon. A self-inflating balloon is located distal to an inflow gap or orifice and distal to a fluid jet emanator, which self-inflating balloon is inflated and expanded by the utilization of internal operating forces consisting of forwardly directed high velocity fluid jet streams and/or entrained thrombus particulate therein. The self-inflating balloon, when inflated, impinges on the wall of the blood vessel to isolate sections of the blood vessel distal and proximal to the inflated balloon in order to prevent flow of thrombus particulate, fluids and the like distal to the self-inflating balloon and to provide a stagnant nonflow region proximal to the self-inflating balloon. The devices of the present disclosure also provide for a uniform spacing of the catheter tube with respect to the thrombus and/or wall of the blood vessel.

An endovascular laser treatment device for causing closure of a blood vessel uses an optical fiber adapted to be inserted into a blood vessel. An inner sleeve is arranged around a distal portion of the optical fiber core such that both distal ends of the inner sleeve and the optical fiber core form an enlarged light emitting face. The enlarged emitting face provides substantially lower power density while providing the same amount of total energy during a treatment session. An outer sleeve arranged around the inner sleeve acts as a spacer to position the light emitting face away from an inner wall of the blood vessel. The enlarged light emitting face and the outer sleeve acting as a spacer reduces the possibility of thermal run-away and device damage, and reduce the possibility of vessel perforations, leading to less bruising, post-operative pain and other clinical complications. In yet another aspect of the present invention, a spacer comprises an inner sleeve and an outer sleeve both arranged around a distal portion of the core to prevent the laser light from traveling laterally and to position the light emitting face away from an inner wall of the vessel. The inner sleeve can be a heat resistive material such as ceramic and the outer sleeve can be, for example, a metallic sleeve to provide structural integrity and strength to the distal section of the treatment device.

Acoustic energy emitted by the transducer is reflected by a highly reflective interface between the balloons. In a cardiac ablation procedure, the ultrasonic energy is focused into an annular focal region to ablate cardiac tissue extending in an annular path along the wall.
