    摘要: A method and apparatus for mechanically lifting the abdominal wall away from underlying abdominal organs for laparoscopic surgery without insufflation. In the method an expansible device is inserted in the abdominal cavity through a small incision in a collapsed state and then expanded into engagement with an extensive area of the abdominal wall. Lifting force is then applied to the device for peritoneal retraction. The device takes the form of mechanical rods or arms and/or balloons. In the balloon embodiments lifting force may be applied externally of the abdominal cavity, or internally of the cavity by balloon inflation. Certain of the balloon embodiments are of an annular or U-shaped configuration and include a membrane for draping the internal organs and/or a centrally located balloon for lateral expansion. The balloons may be provided with an internal endoscope for viewing. The method also provides for laparoscopic gallbladder removal, either to the interior or exterior of the balloons. A needle is provided to laparoscopically pierce and drain the gallbladder. The needle carries a balloon inflatable to grip the gallbladder for retraction and removal.

    摘要: A first inflatable retraction device has a first inflatable chamber and a non-pressurized chamber inside the main chamber. The non-pressurized chamber is expanded by inflating a second inflatable chamber. The non-pressurized chamber enables the main chamber to remain inflated when an aperture is cut in the envelope of the main chamber, through which treatment is carried out. A second inflatable retraction device has an inflatable retractor and a maintainer. The inflatable retractor retracts the organ and the maintainer maintains the organ in its retracted condition after the inflatable retractor is deflated. The maintainer can be inflatable, and can be inside or outside the inflatable retractor. A self-retracting endoscope has an optical assembly with an expandable retractor fitted to its distal end. The distal end of the endoscope is inserted into the body with the retractor in a collapsed condition. The retractor is then expanded to retract organs that would otherwise obstruct the view from the distal end of the optical assembly. After observations are complete, the retractor is returned to its collapsed condition. An insertion tube enables cylindrical objects, such as packaged inflatable retraction devices, to be pulled, instead of pushed, into the body. The additional chamber of an inflatable retraction device having two inflatable chambers is filled with a slurry of a particulate solid in a liquid. The liquid is removed and the additional chamber evacuated to consolidate the particulate solid. This increases the retracting strength of the additional chamber.

    摘要: A method and apparatus for removing a pre-placed prosthetic appliance anchored in place in a bone recess by a cement mantle and conditioning the recess for receipt of a replacement appliance. The preplaced appliance is first pulled from the mantle of hardened cement holding it within the recess, thus leaving a cavity within the mantle. A screw threaded post having nuts threadably engaged therewith at longitudinally spaced locations is then anchored within the cavity with a new mass of cement. Thereafter, the post is threadably disengaged from the nuts, leaving the nuts in place within the new mass of cement. A pulling tool is then successively engaged with the nuts and tensioned to incrementally remove the mantle from the recess.

    摘要: A trocar assembly consisting of a trocar tube subassembly and a separable, interfitting trocar subassembly is described. The trocar tube assembly includes a hollow body having a first front opening in which a trocar tube is mounted, a second rear opening for receiving the trocar subassembly, and a manually pivotable flap valve mechanism mounted within the body. The trocar subassembly includes a head having a central bore in which are mounted the (1) rear ends of an elongated obturator having a pyramidal-shaped piercing head with a triangular-shaped base, and a concentric tubular shield having a triangular shaped front opening for the piercing head that is formed by bevels in its leading edge that have approximately the same slope as the surfaces that define the three sides of the piercing head, and (2) a coil spring backing the shield for biasing the shield between retracted and extended positions. When the two subassemblies are fit together, the obturator and concentric shield extend through the hollow body with the front wall of the head fitting within a recess in the rear wall of the hollow body such that the trocar assembly cannot be rotated without disengaging the two subassemblies. The shield has a slot in its outer wall that is engaged by the flap valve to lock the shield in its extended position. The shield may be unlocked by pivoting the flap valve out of the slot, sliding the trocar assembly rearwardly so that the front wall of the head is clear of the recess in the rear wall of the hollow body, rotating the trocar subassembly to a position in which the flap valve and slot are not aligned and sliding the front wall of the trocar assembly back into the recess in the rear wall of the hollow body.

    摘要: A trocar assembly (70) formed from a trocar tube subassembly (74) and a separable, interfitting trocar subassembly (72). The trocar tube assembly includes a head (112) having a bore (120) into which a trocar tube (116) is fitted and a manually operated slide valve assembly (118) mounted transversely of the bore in the head. The valve has a bore (134) through it that may be aligned with the bore in the head by movement of the valve. The trocar subassembly includes a head (76) that has a bore (92) extending partly through it in which a trocar obturator (82) and a concentric tubular shield (86) are mounted such that the shield is capable of limited axial movement relative to the obturator between a normal, extended position in which the shield effectively covers the piercing tip (84) of the obturator and a retracted position in which the tip is exposed. The obturator and shield are inserted through the bore in the trocar tube subassembly head, the slide valve bore and the trocar tube. The bore in the slide valve has a radial tooth (136) in it that engages a slot (138) in the shield to reversibly lock the shield in its extended position when the slide valve is in its normal position. The valve is depressed manually to disengage the tooth to allow axial movement of the shield or separation of the trocar subassembly from the trocar tube subassembly.
