    摘要: The present invention provides a method and apparatus for assessing ventricular function on a chronic basis using a plurality of electrodes disposed on or about a left ventricle and/or a right ventricle—and optionally, at least one mechanical or metabolic sensor—all operatively electrically coupled to an implantable medical device. The plurality of electrodes are preferably spaced-apart so that at least one electrode is disposed electrical communication with a discrete volume of ventricular tissue. In one embodiment, the discrete volume of tissue is defined by multiple longitudinal and axial planes as known and used in the medical arts. Thus, according to the present invention, at least one electrode couples to appropriate sensing circuitry and essentially provides a localized electrogram (EGM) that, when compared to other EGMs, provides for configurable, localized delivery of therapeutic pacing stimulus, diverse impedance-sensing vectors, various diagnostic information regarding myocardial function and/or anti-tachycardia pacing.

    摘要: An improved medical lead assembly and method of use is provided. The lead assembly includes a lead body, and a spring member positioned adjacent to the lead body. The spring member may be deployed a selectable amount to maintain the lead body in a fixed location within a patient's body. The spring member may be an expandable coil, a mesh structure that is similar to a stent, or any other similar device that may be positioned in a low-profile state during a lead implant procedure. After the lead is positioned at a target destination, the spring member may be deployed an amount that is selected based on the characteristics of the surrounding tissue, including vessel size. According to one aspect of the invention, the lead assembly may provide means for facilitating chronic lead extraction.

    摘要: An electrical connector has a compressible portion that expands to accept an inserted lead and then contracts around the lead to provide both an electrical and a spring-like mechanical connection to the lead. The connector may have a fluted pin that collapses around the lead body when a set screw of the connector port is tightened. Alternatively, a middle segment of a pin may have indentations or slots that collapse around an inserted lead body. The connector can be attached to the lead body after the lead body is implanted in the body of the patient. In lead systems in which the lead body is implanted using a guide catheter, attaching the connector to the lead body after implantation makes removal of the guide catheter much less difficult.

    摘要: An implantable sensing device includes a sensing element and a mounting element. The mounting element has a first end and a second end with a longitudinal axis therethrough. The sensing element is positioned at the first end of the mounting element with the mounting element having a length that is adjustable along the longitudinal axis. The mounting element includes a sleeve having a first open end and a second open end with the longitudinal axis therethrough. The sensing element is positioned in the second open end and includes a lead body connected thereto extending through the second open end. The sleeve includes an outer sleeve member coupled for adjustment along the longitudinal axis with respect to an inner sleeve member. Further, the mounting element may include a flexible element about the first open end extending outwardly relative to the longitudinal axis and a flange element extending outwardly relative to the longitudinal axis from at least a portion of the second end. The device can be implanted in a bone with the flange element and the flexible element for direct or indirect contact with the anterior and posterior surface of the bone. A method of implanting the sensor includes providing a sensing element and drilling a hole in a bone. The hole extends to the intrathoracic cavity. The sensing element is inserted into the hole and positioned in communication with a region in the intrathoracic cavity. The hole is drilled in the manubrium. When the flexible element is inserted into the hole, the flexible element about the first open end collapses towards the longitudinal axis and when the sensor mounting assembly is removed from the hole, the flexible element collapses towards the longitudinal axis during removal. Methods for providing stimulation of a patient to treat respiratory disorders using the sensor are also provided as are stimulation systems for providing stimulation of a patient to treat respiratory disorders.
