摘要: Approximating apparatus for surgical jaw structure is provided which effects substantially parallel approximation of first and second surgical jaw structure to permit substantially parallel working interaction therebetween. The approximating apparatus includes camming structure for engaging at least one movable jaw of the surgical jaw structure to bias that jaw toward or away from a second jaw structure in a substantially parallel manner. A tissue stop may be provided to more easily guide and protect tissue captured by the jaw structure.

摘要: A surgical apparatus for tightening a strap loop to a predetermined tension about split tissue portions includes a frame, an advancing mechanism for tightening the strap loop, a linkage mechanism which severs excess strap material not used in securing the tissue portions together and a control mechanism for controlling operation of the apparatus. The advancing mechanism includes a tensioning lever and a gear mechanism. The gear mechanism includes a tensioning hub and a clutch which releases when the tension in the strap loop exceeds a predetermined value. The control mechanism includes a safety feature for preventing inadvertent activation of the linkage mechanism. A buckle and a method for use with the surgical apparatus is also disclosed. The invention is particularly contemplated for repair of the human sternum when the sternum has been split during surgical procedures.

摘要: Apparatus for applying at least one row of two-part surgical fasteners to body tissue, each surgical fastener having a pronged fastener portion for piercing body tissue, and an aperture retainer dimensioned and configured for engaged reception of the pronged fastener portion in interference fit therewith for gripping the body tissue therebetween. The apparatus includes means for holding a plurality of the fastener portions in generally aligned relation, means spaced from the fastener portion holding means for gripping body tissue therebetween and for releasably holding a plurality of the retainers in generally aligned relation and positioned opposite the fasteners when the body tissue is positioned therebetween. Means is provided for sequentially advancing the pronged fastener portions toward the apertured retainers to cause the fastener portions to pierce the body tissue and to be received within the apertures of the retainers in engaged interference relation so as to cause the fastener portions and the retainers to be engaged while gripping the body tissue therebetween. In the preferred embodiment a knife edge is provided to cut the body tissue midway between opposed pairs of rows of the fasteners as the fasteners are applied. A method for applying two spaced apart pairs of rows of the fasteners to body tissue on the apparatus of the invention is also disclosed.

摘要: A detachable anvil assembly for use with an apparatus for performing circular anastomosis of tubular organs includes an elongated anvil rod having proximal and distal end portions and an anvil head mounted to the distal end portion of the anvil rod. The distal end portion is pivotally mounted and is adapted to pivot approximately plus or minus ninety degrees relative to a longitudinal axis defined by the rod. An elongated delivery member detachably receives the anvil assembly and facilitates delivery of the anvil assembly to the operative site. The pivoting feature of the distal end reduces the transverse profile of the assembly which consequently facilitates introduction and advancement of the anvil assembly within the tubular organ.

摘要: A bioabsorbable surgical fastener comprising a fastener member and an interlocking retainer member. The retainer member has longitudinally extending slots on the lateral sides of the retainer for allowing transverse expansion of the columnar members into which the prongs of the fastener member are locked. Fins on both the lateral and transverse sides of the prongs provide extra strength, and stability during implantation. Stopping surfaces prevent the fastener's barbed tip from exiting the opposite side of the retainer.

摘要: An apparatus for endoscopic application of surgical staples adapted to attach surgical mesh to body tissue in laparoscopic hernia surgery includes an elongate member defining a longitudinal axis, a cartridge, an anvil, and a control member located in a proximal region of the elongate member. The cartridge is removably supported on a pivotal support system at the distal end portion of the endoscopic section with each staple being configured and adapted to attach the mesh to the body tissue. The anvil provides for individually closing each staple to encompass at least a portion of the surgical mesh and to penetrate the body tissue in a manner to attach the portion of the mesh to body tissue. The control member is axially movable along the longitudinal axis to articulate the cartridge. The control member is also rotatable to rotate the cartridge.

摘要: An apparatus is disclosed for endoscopic application of surgical staples adapted to attach surgical mesh to body tissue in laparoscopic hernia surgery. The apparatus includes a frame, and a generally elongated endoscopic section connected to the frame and extending distally therefrom. A staple storage cartridge is removably supported on a pivotal support system at the distal end portion of the endoscopic section with each staple being configured and adapted to attach the mesh to the body tissue. An elongated pusher system formed of several assembled components and extending from the frame to the endoscopic section is provided for individually advancing at least one staple at a time distally for positioning adjacent the surgical mesh and the body tissue. The pusher system also includes a trigger system to actuate the pusher. The trigger system is provided with perceptible tactile sensing means to indicate when the legs of the staple being advanced are exposed so as to be visible to the user for positioning and orientation purposes. Anvil means provides for individually closing each staple to encompass at least a portion of the surgical mesh and to penetrate the body tissue in a manner to attach the portion of the mesh to the body tissue. Projecting distally of the cartridge support system is a pair of legs which are dimensioned and configured to engage the staple during closure to prevent unwanted roll or deformation outside of the plane of the staple.

摘要: A surgical instrument is provided for use in endoscopic or laparoscopic procedures. The instrument includes a handle portion, an endoscopic portion extending from the handle portion, an articulating section pivotably connected to a distal end portion of the endoscopic portion, and a retractor assembly operatively associated with the articulating section. Structure is provided for manipulating the articulating section relative to the longitudinal axis of the endoscopic portion within an angular degree of rotation. An injection port may also be provided to deliver fluids through the endoscopic portion to the operative site.
