    摘要: The invention provides a delivery system for placing devices in atrial appendages. The system includes a catheterization apparatus having a tubular structure with one or more nested tubes, wires, and shafts. The tubes establish a passageway for moving a device through a body's vasculature and heart into an atrial appendage. An expandable positioning guide is disposed on the distal end of a tube passing through the apparatus. The positioning guide is expanded in situ to engage atrial wall surfaces proximate to the atrial appendage for mechanical support. The deployed positioning guide mechanically stabilizes the device delivery passageway. A shaft passing through the tubes transports the device through the passageway to the atrial appendage.The system includes catheterization apparatus for transseptal delivery of the devices. An outer tube or sheath in the apparatus is used to penetrate and traverse the septum. An expandable securement device is disposed toward an end of the sheath. The securement device is expanded in situ to engage surface portions of the traversed septum to restrain further inadvertent movement of the septum-traversing sheath.

    摘要: A membrane applied to the ostium of an atrial appendage for blocking blood from entering the atrial appendage which can form blood clots therein is disclosed. The membrane also prevents blood clots in the atrial appendage from escaping therefrom and entering the blood stream which can result in a blocked blood vessel, leading to strokes and heart attacks. The membranes are percutaneously installed in patients experiencing atrial fibrillations and other heart conditions where thrombosis may form in the atrial appendages. A variety of means for securing the membranes in place are disclosed. The membranes may be held in place over the ostium of the atrial appendage or fill the inside of the atrial appendage. The means for holding the membranes in place over the ostium of the atrial appendages include prongs, stents, anchors with tethers or springs, disks with tethers or springs, umbrellas, spiral springs filling the atrial appendages, and adhesives. After the membrane is in place a filler substance may be added inside the atrial appendage to reduce the volume, help seal the membrane against the ostium or clot the blood in the atrial appendage. The membranes may have anticoagulants to help prevent thrombosis. The membranes be porous such that endothelial cells cover the membrane presenting a living membrane wall to prevent thrombosis. The membranes may have means to center the membranes over the ostium. Sensors may be attached to the membrane to provide information about the patient.

    摘要: Instrumentation for percutaneous delivery of blood filtration devices to atrial appendages includes a curved access sheath and a delivery tube. A compressed filter device attached to a tether wire is loaded in the delivery tube. The access sheath and the delivery tube can be mechanically locked and moved together to place the device in a suitable deployment position. The device is deployed by expelling it from the delivery tube either by retracting the delivery tube over the tether wire, or by moving the tether wire forward through the delivery tube. A filter membrane in the deployed device extends across the appendage ostium to filter blood flow through the ostium. The filter membrane is configured to present a flat surface to atrial blood flow past the ostium.

    摘要: A system and method for retrieving an implantable device includes a delivery catheter, a recapture section, and a sheath. The delivery catheter has a proximal end and a distal end. The recapture section is axially extendable from the distal end of the delivery catheter. The sheath has a proximal end and a distal end and a lumen sized to receive the delivery catheter. A portion of the lumen of the sheath is actuatable from an enlarged inside diameter to a reduced inside diameter to apply an inwardly directed force to the recapture section. The delivery catheter can be actuated with respect to the sheath to extend or retract the recapture section with respect to the delivery catheter.
