A valve assembly adapted for introduction of an elongated object into a patient's body having a first valve formed of a resilient material and defining an aperture for reception of the object, the aperture being configured and dimensioned such that insertion of the object into the aperture will cause the resilient material defining the aperture to resiliently engage the outer surface of the object in a fluid tight manner. A second valve is positioned adjacent and distal of the first valve in general alignment therewith, whereby the second valve defines an aperture in general alignment with the aperture of the first valve, and is formed of a flexible material at least in the region defining the aperture. A pair of manually operable clamps are provided to selectively permit the aperture of the second valve to be opened or closed so as to permit entry of the object such that the object first passes through the first valve and then the second valve prior to entry into the patient's body.

Apparatus is disclosed for applying surgical fasteners to body tissue along an arcuate path. Each surgical fastener may be in the form of a two-part fastener having a pronged fastener portion for piercing body tissue, and an apertured retainer dimensioned and configured for engaged reception of the pronged fastener portion in interference fit therewith for gripping the body tissue therebetween. Alternatively, the fasteners may be in the form of deformable staples. The apparatus preferably includes an arcuately shaped member for holding a plurality of the fastener portions in generally aligned relation, a correspondingly arcuately shaped member spaced from the fastener portion holding member for gripping body tissue therebetween and for releasably holding a plurality of the retainers in generally aligned relation and positioned opposite the fasteners when the body tissue is positioned therebetween. A fastener driver is provided for sequentially advancing the pronged fastener portions toward the apertured retainers to cause the fastener portions to pierce the body tissue therebetween. A knife may be provided to cut the body tissue adjacent the fasteners as the fasteners are applied.

A surgical apparatus is disclosed for attaching adjacent end portions of cutaneous body tissue wherein a pair of opposed jaws have means for moving the jaws toward and away from each other. Body tissue engaging means in the form of sharp tip members extend from each jaw and toward the other so as to engage the opposed portions of cutaneous tissue when the jaws are positioned thereagainst and moved toward each other. The jaws and the tips are arranged to cause the cutaneous tissue to assume an irregular shape at the interface whereby an elongated rod-like member, preferably rectangular in cross section, may be directed generally medially of the interface of the tissue to attach the opposed portions to thereby permit healing. The elongated member may have indentations to improve retention within the tissue. A tapered distal end of the elongated member has a pointed tip to facilitate penetration into the tissue.

An apparatus is disclosed for endoscopic application of surgical staples for fastening body tissue, especially vascular tissue. The apparatus includes a frame and an elongated endoscopic section connected to the frame and extending distally therefrom. A staple storage cartridge cooperates with the distal end of the endoscopic section and is selectively pivotal by the user. A staple firing mechanism advances an individual staple against an anvil for staple closure. Means are provided to prevent incomplete staple firing. Relative motion between the staple cartridge and the anvil clamps tissue to be stapled therebetween.The endoscopic section may be adapted for rotation about the longitudinal axis of the apparatus. Gaseous seal means located in the endoscopic section prevent escape of insufflating gas through the apparatus.

The actuating shaft mechanism is longitudinally movable in a first direction by a pivotal movement of the pivot handle for manipulating the laparoscopic tool. A detent carried by the casing allows movement of the shaft longitudinally in the first direction while restraining longitudinal movement of the shaft in a second opposite direction. A thumb wheel attached to the actuating shaft mechanism allows the actuating shaft mechanism to be rotated for further manipulating the laparoscopic tool.

An apparatus is disclosed for endoscopic application of surgical staples adapted to attach surgical mesh to body tissue in laparoscopic hernia surgery. The apparatus includes a frame, and a generally elongated endoscopic section connected to the frame and extending distally therefrom. A staple storage cartridge is removably supported on a pivotal support system at the distal end portion of the endoscopic section with each staple being configured and adapted to attach the mesh to the body tissue. An elongated pusher system formed of several assembled components and extending from the frame to the endoscopic section is provided for individually advancing at least one staple at a time distally for positioning adjacent the surgical mesh and the body tissue. The pusher system also includes a trigger system to actuate the pusher. The trigger system is provided with perceptible tactile sensing means to indicate when the legs of the staple being advanced are exposed so as to be visible to the user for positioning and orientation purposes. Anvil means provides for individually closing each staple to encompass at least a portion of the surgical mesh and to penetrate the body tissue in a manner to attach the portion of the mesh to the body tissue. Projecting distally of the cartridge support system is a pair of legs which are dimensioned and configured to engage the staple during closure to prevent unwanted roll or deformation outside of the plane of the staple.

Approximating apparatus for surgical jaw structure is provided which effects substantially parallel approximation of first and second surgical jaw structure to permit substantially parallel working interaction therebetween. The approximating apparatus includes camming structure for engaging at least one movable jaw of the surgical jaw structure to bias that jaw toward or away from a second jaw structure in a substantially parallel manner. A tissue stop may be provided to more easily guide and protect tissue captured by the jaw structure.
