摘要: Apparatus and methods are provided for placing and advancing a diagnostic or therapeutic instrument in a hollow body organ of a tortuous or unsupported anatomy, comprising a handle, an overtube, a distal region having an atraumatic tip. The overtube may be removable from the handle, and have a longitudinal axis disposed at an angle relative to the handle. The overtube may be selectively stiffened to reduce distension of the organ caused by advancement of the diagnostic or therapeutic instrument. The distal region permits passive steering of the overtube caused by deflection of the diagnostic or therapeutic instrument while the atraumatic tip prevents the wall of the organ from becoming caught or pinched during manipulation of the diagnostic or therapeutic instrument.

摘要: Apparatus and methods for cooling selected regions within a body are described herein. An implantable cooling system is used to cool regions of the brain, spinal cord, fibrous nerve bodies, e.g., vagus nerve, etc. down to about 30° C. to diminish nerve impulses which control seizures or chronic pain. The system includes an implantable unit containing a pumping mechanism and/or various control electronics. It also has a heat exchanger attachable to a tubular body organ, such as the superior vena cava or the inferior vena cava, through which the heat is effectively dissipated. Also included is a heat pump such as a Peltier junction configured to be placed into contact with the region of tissue to be cooled. The heated portion of the Peltier junction is cooled by a liquid heat transfer medium which absorbs the heat from the junction and dissipates it into the tubular body organ.

摘要: Apparatus and methods for intraoperatively performing surgery to create transmural channels in tissue, for example, as in transmyocardial revascularization, are provided using a device including a mechanical end effector and means for stabilizing the end effector in contact with the tissue. The end effector is adapted to cooperate with a source of suction to evacuate tissue severed during the channel forming process, and may optionally include an electrode for cauterizing the tissue surrounding the channel.

摘要: This invention is a device (10) for releasing a therapeutic appliance, such as an embolic coil (12) inside an aneurysm (16). The device includes a coupling (24) of a shape memory alloy which is responsive to a change in temperature beyond a predetermined transformation point so as to change the shape of the coupling from a first configuration, wherein the coupling receives and holds the proximal end (72) of the coil, to a second configuration wherein the coil can be released from the coupling. An energy receiver (46) is operably associated with the coupling to heat the coupling to a temperature above the transformation point with laser or electrical energy received from an external source.

摘要: An apparatus and method for human female sterilization. The apparatus includes an occlusive device configured from an elongate tubular member having a central lumen and a flange formed at the proximal end. The occlusive device is formed with a plurality of flexible ribs configured to provide a plurality of seals within the interstitial portion of a Fallopian tube. A valve member is preferably disposed within the lumen of the tubular member. The occlusive device may further include a plurality of copper rings disposed between the ribs of the tubular member, or ribs may be coated with a material containing copper. The method includes placing the occlusive device within a delivery catheter having a lumen and placing the distal end of the delivery catheter within the uterus. After the occlusive device and distal end of the delivery catheter are placed in the Fallopian tube, the occlusive device is removed from the delivery catheter and ejected into the Fallopian tube. In addition, a guide wire may be inserted through the vagina and into a Fallopian tube of the uterus such that the occlusive device and the delivery catheter are threaded over the guide wire.

摘要: Apparatus and methods for performing surgery within a hollow-body organ are provided. A catheter is provided having a longitudinal axis and an end region carrying an end effector. The end region is movable to a series of positions along the longitudinal axis with a selectable orientation relative to the longitudinal axis. The catheter includes elements for stabilizing the end region of the apparatus within a hollow-body organ, and for counteracting reaction forces developed during actuation of the end effector. The end effector provides for tissue cutting and aspiration of severed tissue from a treatment site.

摘要: Cervical dam for capturing liquid draining from uterus during gynecological procedures while simultaneously providing an aperture through the cervical dam for insertion and manipulation of a medical device. The aperture may be a preformed slit elastomeric aperture, a duck billed valve, a multi-disc elastomeric valve, or a universal seal. The cervical dam includes a drainage port through the body of the cervical dam, a drainage tube attached to the drainage port, and a handle, which may be detachable, affixed to the cervical dam for placement, manipulation and removal of the cervical dam. Also disclosed are methods for performing a gynecological procedure by placing a cervical dam over the cervical canal inserting a medical instrument through the cervical dam into the cervical canal, performing a procedure, and withdrawing the medical instrument, and by determining the amount of fluid input to a patient during the procedure and the amount drained from the patient during and after the procedure, and comparing the two.

摘要: An apparatus for thermal coagulation and more particularly to a system for applying heat to the endometrium of a human uterus of the type comprising a heated balloon catheter having a reciprocating piston for improved circulation of fluid within a distensible bladder. The present invention includes a heated balloon catheter having a fluid delivery tube connected to a distensible bladder. The catheter further includes a reciprocating piston which moves along a linear path within the distensible bladder to cause vigorous fluid agitation therein. The agitation caused by the piston efficiently mixes the fluid within the balloon without requiring hot fluid to circulate through the fluid delivery tube.

摘要: Methods and apparatus for securing and deploying tissue anchors are described herein. A tissue manipulation assembly is pivotably coupled to the distal end of a tubular member. A reconfigurable launch tube is also pivotably coupled to the tissue manipulation assembly, which may be advanced through a shape-lockable endoscopic device, a conventional endoscope, or directly by itself into a patient. A second tool can be used in combination with the tissue manipulation assembly to engage tissue and manipulate the tissue in conjunction with the tissue manipulation assembly. A deployment assembly is provided for securing engaged tissue via one or more tissue anchors, the deployment assembly also being configured to disengage the anchors endoluminally or laparoscopically.
