    摘要: Clips of a self-closing type are used in annuloplasty procedures. Each clip is generally U-shaped with two end points separated from each other when it is constrained to be in an open configuration, but tends to coil up to assume its naturally closed configuration if the constraint is removed. A plurality of such clips in open configurations penetrate the tissue around the annulus circumferentially and then the constraint keeping them in the open configuration is removed such that they pull the tissue together between their two end points and this tends to reduce the diameter of the annulus. Such clips may be deployed each in the form of a clip assembly, having at least one of its end points connected to a needle through a flexible member and a release mechanism by which the clip can be easily released. The needle is caused to penetrate the tissue at one position and to come out therefrom at another circumferentially separated positions. Alternatively, a clip delivery device may be used with a plurality of such clips loaded to a clip-holder serving to keep them in open configurations. A pusher pushes the loaded clips out of the device one at a time. Self-closing clips can be used efficiently also in ring annuloplasty and valve replacement procedures.

    摘要: A clip of a self-closing type is used for valve repair. The clip is generally U-shaped with two end points separated from each other when it is constrained to be in an open configuration, but tends to coil up to assume its natural closed configuration if the constraint is removed. At least one end point is connected through a suture to a tissue-penetrating needle. A needle holder has an outer tube and an inner member which has a slit at the front and is slidable inside the outer tube. They are designed such that the slit can grab the needle tightly or release it as the inner member is moved backward or forward with respect to the outer tube. With the needle secured at the front, the needle holder is passed through a cannula inserted through an incision and the needle penetrates the leaflets such that the clip can be positioned with its end points hooked to the pair of leaflets to be stitched together. If the clip is released from the suture thereafter, it tends to coil up, reducing the distance between the end points and pulling the leaflets together. A double-arm clip assembly with each of the end points of the clip attached through a suture to a separate needle may be similarly used for valve repair. Such a double-arm clip assembly may contain two of such clips mutually connected by means of a flexible connector such that two leaflets are held together by the connector, with the two clips each anchored to a corresponding one of the leaflets.

    摘要: An innovative method of playing a modified poker game with seven related variations featuring a novel method of tournament mechanics, through an electronic device or in a casino, known as “Tennessee Hold 'Em® Tournaments”, which promotes strategic decision making and greater player participation is defined. Three to nine players are permitted per table. Three players sitting to the left of the Dealer (Button) furnish blind bets. Five phases of card distribution and progressively increasing betting rounds are defined. After the third card distribution, players decide whether to hold all hole cards, or discard one or two hole cards. Next, all discards are revealed, giving players insight into opponents' hands. Players choosing to hold 4 hole cards are excluded from using the fourth, final community card. Players choosing to hold 5 hole cards are excluded from using the third and fourth community cards. Eliminating complex ‘side pots’, an innovative method of “loaning” additional chips, known as ‘overdrawn chips’ (ODC) allows players who do not have enough chips to continue betting and complete in that hand, providing a last chance to recover or be eliminated. The Tournament is finished when 2 players remain. Winner and runner up chip counts are totalled, and the winner's chip count is divided by the total chip count to determine the percentage of the Tournament Pot awarded to the winner. Remainder of the Tournament Pot is awarded the runner up.
