    摘要: A system makes payments from each payer of a community of payers to each vendor of a community of vendors, assesses a variable transaction fee to each vendor, and provides a variable rebate to each payer. The transaction fee is “variable” because it may be varied: i) amongst vendors (the same payer may use different fee rates for different vendors); and ii) amongst payers (the same vendor may be subjected to different rates, based on the payer making the payment). The variable rebate is based on the total transaction fee applicable to payments made by the payer during a period of time. The rebate may be the sum of: i) the product of the total transaction fee multiplied by a base rebate percentage; and ii) the product of that portion of the total transaction fee in excess of an accelerator threshold multiplied by an accelerated rebate percentage.

When a request for a destination location for display data is received, a determination is made as to whether the data will require further processing prior to display. If further processing is needed, a location of an external application buffer is provided. If no further processing is needed, a frame buffer is provided. Thus, improved efficiency is achieved, while still providing functionality for processing occlusions and/or composited sections of the display data.

    摘要: The optimal location for delivering pacing pulses to a given cardiac chamber (such as the left ventricle) is determined by identifying the last electrically or mechanically activated site within the chamber. For example, the last site within the left ventricular myocardium to depolarize during an intrinsic ventricular contraction triggered by an initial atrial depolarization is identified as being the optimal pacing location for the left ventricle. A pacing electrode is then implanted as close as possible to that location. The technique is particularly effective for use in identifying optimal epicardial pacing locations for mounting satellite pacing devices for use in delivering cardiac resynchronization therapy. However, the techniques may also be applied to identify locations for mounting endocardial electrodes as well. Capture thresholds may also be taken into account in determining the optimal location. Examples are described for both master/satellite pacing systems as well as biventricular single device systems.

    摘要: An implantable cardiac lead system suitable for placement in the coronary sinus region of the heart. The lead system comprises a lead having two or more non-helical bends in its distal portion. The two or more non-helical bends cooperate to prevent the lead from dislodgment or displacement inside the coronary sinus. The lead system may further comprise a stylet suitable for steering the lead into at least one of the coronary sinus vein, great cardiac vein, left marginal vein, left posterior ventricular vein, and small cardiac vein. The stylet is tapered in its distal portion to provide enhanced maneuverability and steerability inside the coronary sinus region. The lead system may also comprise an introducer which aids in introducing the lead into the heart.
