An endoscopic instrument has a distal assembly removable from a proximal handle. The distal assembly includes a tube carrying an end effector and a push rod coupled to the end effector and slidable through the tube. The proximal handle includes a tube sleeve for receiving the tube, an actuator and a latch for coupling the push rod to the actuator. The tube sleeve is provided with a tube lock for holding the tube securely in place and the tube is provided with a circumferential groove for engaging the lock. The latch is spring loaded, hinged, and has an inclined surface for quick coupling with the push rod. The latch also has a mechanism for uncoupling the latch from the push rod. The push rod is provided with a mating tip which engages the latch so that the actuator causes reciprocal movement of the push rod within the tube to operate the end effector. An axially movable collar on the tube sleeve is used in unlatching, and in one arrangement is also used for rotating the distal assembly.

An improved use counter for endoscopic instruments is mounted in the ferrule of the distal portion of an endoscopic instrument having separable proximal and distal portions. The counter includes a ratchet wheel having inner and outer teeth and a bimetallic coil having a fixed end and a free end. The fixed end of the bimetallic coil is attached to a hub portion of the ferrule and the free end of the bimetallic coil engages the inner teeth of the ratchet wheel. An inwardly extending tooth in the ferrule engages the outer teeth of the ratchet wheel to prevent backward movement of the ratchet wheel. The outer surface of the ratchet wheel is also provided with incremental indicia viewable through a window in the ferrule. As the bimetallic coil is heated and cooled when the instrument is sterilized, the free end of the coil engages and advances one tooth of the ratchet wheel. Movement of the free end is preferably limited by stops. A spring biased locking and disabling pin is mounted in the ferrule and engages a stepped width portion of the ratchet wheel. The pin prevents advancement of the ratchet wheel during shipping and disables the instrument after a predetermined number of sterilizations.

An apparatus for counting the number of times a temperature cycle occurs includes an indicator having sequential indicia and a temperature responsive member for indicating a next one of the indicia. Mechanical, fluid mechanical and electronic versions of the invention are disclosed. The uninsulated proximal end of the tube is mounted in a conductive bushing which rotates within a nonconductive handle. The proximal end of the push rod extends beyond the conductive bushing and a rubber insulator is fitted between the bushing and the push rod. A nonconductive actuating lever is hingedly attached to the handle and the push rod is rotatingly coupled thereto. A pair of Y-shaped electrical wiper contacts are insert molded in a nonconductive member mounted in the handle and respectively contact the bushing and uninsulated portion of the push rod on either side of the rubber insulator. A bipolar cable insert molded in the nonconductive member is coupled to these contacts and to a source of current.

The instrument has a fluid chamber sealed at its proximal end with a slit valve for receiving the probes and connected at its distal end to a cannula through which the inserted probes extend. The fluid chamber is selectively provided with suction and/or irrigation and includes an electrical contact for supplying voltage to an inserted electrosurgical probe. Depending on the probe configuration suction and/or irrigation is provided in an annular space between the probe and the cannula or through the probe. An endoscopic multiple sample bioptome includes a hollow outer member and an axially displaceable inner member extending therethrough. The proximal ends of the outer and inner members are coupled to an actuator for axially displacing one relative to the other. The distal end of the outer member is coupled to one of a cylinder having a sharp distal edge and a jaw assembly, while the distal end of the inner member is coupled to the other. The jaw assembly includes a pair of opposed, preferably toothed jaw cups each of which is coupled by a resilient arm to a base member. The arms are bent to urge the jaws away from each other. The base member is mounted inside the cylinder and axial movement of the jaw assembly and cylinder relative to each other draws the arms into the cylinder and brings the jaw cups together in a biting action. When the outer member is coupled to the jaw assembly and the inner member is coupled to the cylinder, the distal end of the inner member is preferably coupled to the cylinder by a bent end which is welded to a hole in the cylinder wall. One of the jaws is preferably larger and has a pair of proximal wings. The wings and an upper edge portion of the larger jaw are bent radially inward and enter the smaller jaw when the jaws are closed.

An endoscopic scissors includes a clevis having a relatively large diameter bore in its first arm and relatively smaller diameter bore in its second arm. A pair of scissor elements having mounting/pivot holes are mounted between the clevis arms. Either the small diameter bore in the second clevis arm or the mounting/pivot hole in the scissor element closest to the second clevis arm is threaded. A stepped diameter axle pin having a threaded portion is inserted with its smaller end entering the larger diameter bore of the first clevis arm. The threaded portion of the axle pin threadably engages either the threaded bore in the second clevis arm or the threaded mounting/pivot hole of the scissor element closest to the second clevis arm. In one embodiment, the bore in the second clevis arm is not threaded and the threaded portion of the axle pin engages only the threaded threaded mounting/pivot hole of the scissor element closest to the second clevis arm. Both ends of the axle pin float in respective clevis arm bores. In another embodiment where the scissor element closest to the second clevis arm is kept stationary, the threaded portion of the axle pin engages the threaded bore in the second clevis arm. The larger end of the axle pin floats in the large diameter bore of the first clevis arm. In a third embodiment, a washer is placed between the second clevis arm and the scissor element closest to it and the axle pin engages the threaded bore in the second clevis arm.
