    摘要: Pieces of tissue are fused together and simultaneously cut by compressing the pieces together at an interface, delivering an impulse of electrical power which is converted into sufficient thermal energy to fuse the pieces together at the interface and simultaneously cut the fused tissue pieces along a well-defined linear path. The impulse, and the fusion and the simultaneous cutting, occur within a preferable time of 1.5-2.0 seconds but no greater than 4.0 seconds. The temperature of the thermal energy is regulated between 220° C.-320° C. The force applied on the tissue pieces is sufficient to fuse the tissue at the interface followed by further compression to cut the fused tissue along the linear path as a result of the thermal energy and a zero distance gap between jaws which compress the tissue pieces. The jaws have smooth working surfaces with an Ra of 0.15 or less microns up to 0.40 microns.

    摘要: A cutting device for performing a surgical procedure includes an elongated shaft extending between a proximal end and a distal end, a first cutting element, and a second cutting element. The elongated shaft of the cutting device includes an inner surface defining a passageway. The first cutting element extends from the distal end of the elongated shaft so as to contact tissue. The second cutting element is disposed at least partially within the passageway and extends from the distal end of the elongated shaft. The second cutting element is configured for retractable extension beyond the distal end of the elongated shaft so as to contact tissue. The cutting device also includes a deployment mechanism disposed with the proximal end of the elongated shaft for engaging and actuating the second element.

    摘要: Endoscopic bipolar scissor blades are formed in a laminate assembly of an outer electrically conductive layer, an intermediate electrically insulating layer, and an inner face layer which is a coating of titanium dioxide, chromium dioxide, or zirconium dioxide. In one embodiment, the conductive layer is a metal blade, while the insulating material is a ceramic which is fixed to the metal blade. In a second preferred embodiment, the insulating layer and the inner face layer are applied simultaneously by coating the metal blade with a mixture of ceramic and titanium dioxide, chromium dioxide, or zirconium dioxide. In a third embodiment, the insulating layer is a molded ceramic blade, and the electrically conductive layer is metalized or otherwise fixed to the ceramic blade. In all embodiments, the cutting edges and shearing surfaces are insulated from the electrodes, and no short circuit can form between the electrodes even though the cutting edges and shearing surfaces are metal. Each of the embodiments may be applied to straight or curved scissor blades. A bipolar endoscopic instrument utilizing blades according to the invention is also disclosed.

    摘要: Endoscopic biopsy forceps jaws having an outer edge which may be generally U-shaped, circular, or oblate circular are disclosed with teeth which extend completely around the outer edge. The teeth may take any of several arrangements. In a first arrangement, the teeth and valleys are formed with two substantially rectangular surfaces which meet at substantially horizontal vertex and nadir lines. In a second arrangement, each tooth has a vertex or peak located upward and inward from the outer edge, and is defined by a substantially triangular inner surface, a substantially quadrilateral base, and a surface which extends outward and downward from the vertex of the triangular surface to the outer edge of the jaw. Valleys between the teeth have two inclined surfaces which intersect along a line which extends inward and downward from the outer edge of the jaw. In a third arrangement, the peak of each tooth is defined by the vertex of a pyramid having four faces and valleys between these teeth have three substantially flat surfaces which intersect at a vertex which lies downward and inward from the outer edge of the jaw. Alternate embodiments of all arrangements include providing a lip around the outer portion of the outer edge so that the teeth and valleys are spaced farther in from the outer surface of the jaw cup.
