The valve assembly includes a sealing gasket assembly providing a fluid tight seal before instrument insertion, and is configured and dimensioned for accommodating an instrument and providing a substantial fluid tight seal after insertion of an instrument. The valve assembly may further include a deformable sealing member having a substantially central aperture for accommodating the instrument. The sealing member provides a substantial seal about the instrument when the instrument is passed therethrough impeding the egress of fluids and gasses through the valve assembly.

An apparatus is disclosed for endoscopic application of surgical staples adapted to attach surgical mesh to body tissue in laparoscopic hernia surgery. The apparatus includes a frame, and a generally elongated endoscopic section connected to the frame and extending distally therefrom. A staple storage cartridge is removably supported on a pivotal support system at the distal end portion of the endoscopic section with each staple being configured and adapted to attach the mesh to the body tissue. An elongated pusher system formed of several assembled components and extending from the frame to the endoscopic section is provided for individually advancing at least one staple at a time distally for positioning adjacent the surgical mesh and the body tissue. The pusher system also includes a trigger system to actuate the pusher. The trigger system is provided with perceptible tactile sensing means to indicate when the legs of the staple being advanced are exposed so as to be visible to the user for positioning and orientation purposes. Anvil means provides for individually closing each staple to encompass at least a portion of the surgical mesh and to penetrate the body tissue in a manner to attach the portion of the mesh to the body tissue. An umbilical cord clip and applicator is disclosed for clamping a maternal side of an umbilical cord, applying an umbilical cord clip to a fetal side of the umbilical cord and severing the cord intermediate the clamped maternal and fetal sides of the umbilical cord. The applicator includes a first arm having a first clamping surface, a second arm movably affixed to the first arm and having a second clamping surface and structure associated with at least one the first and second arms for mounting an umbilical cord clip thereon. The applicator also includes a recessed severing mechanism operatively associated with the first and second arms and disposed intermediate the first and second clamping surfaces and the mounting structure. The severing mechanism is automatically actuable upon closure of the first and second arms about an umbilical cord to sever the cord at a location between the first and second clamping surfaces and the clip. The umbilical cord clip includes a first leg having a first clamping surface and a second leg movably affixed to the first leg and having a second clamping surface. A lock device is associated with the first and second legs for maintaining the first and second legs in a closed position about the umbilical cord. Attachment structure is also associated with at least one of the first and second legs. The attachment structure is releasably engagable with the mounting structure on the applicator to hold the umbilical cord clip on the applicator.
