    摘要: An anterior cruciate ligament or soft tissue tensioning and knee displacement measuring device and method for its use to apply a tensile stress into a repaired or replaced graft and to maintain tension on that graft while an interference screw, or like fastener, is turned into a tunnel section end, containing the graft or sutures connected to the graft, wedging it against the tunnel wall, and, during application of the tensile stress, for determining the distance that the knee is displaced. The invention includes a tensioning device having, as a body, a pair of straight rails with a cross member secured across the rails rear or distal ends with forward or proximal rail ends connected to mounting tubes that are for fitting into pins as have been mounted into to extend from a bone cortex, alongside the tunnel section end, and including a trolley maintained to travel between the body rails. The trolley includes a mount arranged to receive and maintain sutures that extend out from the tunnel end, with trolley movement provided by turning a connected rod turned through the cross member to impart a tensile force into the ligament graft, with the amount of the applied force measured by a slide indicator arranged with the trolley. Additionally, the invention includes the knee displacement measuring device for measuring displacement during application of a tensile stress to sutures attached to the ligament graft.

    摘要: A bone anchor and method for its use in an arthroscopic and open soft tissue surgical procedure to mount a ligament graft, or the like, under tension and at variable lengths in a tunnel section that is formed through a bone or bones. The bone anchor embodiments of the invention each have a body for mounting into and/or adjacent a bone cortex surface in a tunnel section to allow at least one suture or shaft that has been fitted through the bone anchor, and is attached to the ligament graft to be pulled through the bone anchor. The suture or shaft to pass across a movable clamp fitted within the body to move so as to allow the suture or shaft to be pulled freely through the bone anchor distal, but will clamp against the suture or shaft, prohibiting its being pulled back therethrough. A pulling of the suture or shaft through the seated bone anchor applies a tension and sets a required length to a graft whose opposite end is secured, with the clamp locking against a section of that suture or shaft, to maintain that applied tension. Whereafter, the excess suture or shaft can be cut away, permanently mounting the graft.

    摘要: An anchor driver for mounting a ligament anchor and turning it into a bone, which ligament anchor includes a suture attached to a rear end that is maintained in a driver body until the anchor driver is pulled off of the ligament anchor seated in the bone. The driver body is preferably a rigid straight tube that includes an anchor mount disposed on a forward end whereto the ligament anchor is fitted. So arranged, a suture, that is disposed on the ligament anchor coupling end, will extend therefrom as a suture strand or strands that are fitted through the anchor mount, to pass through the driver body and are then bent to double back upon themselves and fitted back into the driver body. For maintaining the suture strand or stands in a straight attitude, in one embodiment a resilient cap is provided that has a hole or slot formed therein wherethrough the suture strand bend or bends are fitted into a cavity within the cap, which cap is then fitted over the driver body open end. With, in another embodiment, a grommet and thin walled tube are disposed end to end in the driver body wherethrough the suture strand or strands are fitted that are then bent to double back alongside the tube, and are maintained within the driver body, which driver body then receives a cap fitted over an open rear end thereof.

The groove intersects, on its proximal end, an inlet end of a second curved passage that extends through and exits the driver body surface on the opposite side to the longitudinal groove and first curved passage end, which first curved passage, longitudinal groove and second curved passage are to receive the suture threaded therethrough. The invention provides several embodiments of arrangements for maintaining the threaded suture alongside the driver body, and provides, on the driver body proximal end, a drill coupling arrangement that is to fit into and be turned by a conventional drill, T-handle, or the like, for seating the anchor into a bone material.

The bone fixator includes threaded footing member for mounting in a bone tunnel, that includes a longitudinal opening formed therethrough, and a ligament carrying member that has a forward of proximal portion for fitting through the threaded footing member longitudinal opening and including a coupling arrangement for securing the ligament carrying member in the threaded footing member. The ligament carrying member further includes as a rear or distal portion that incorporates a ligament mounting section, that, in one embodiment, is an eyelet section to receive a ligament graft fitted therethrough and folded upon itself and in another embodiment is a flat section having a center longitudinal axis wherefrom at least one and preferably a pair of spaced apart pointed posts extend at essentially right angles that include serrations or downwardly sloping teeth formed along at least one side of each post, the post or posts for passing through transverse holes formed in an end of a ligament graft, such as a bone end of a bone tendon ligament graft, with the ligament graft bone end urged onto the mounting plate posts axially mounting it thereto. The ligament carrying member with axially mounted ligament graft are guided in the bone tunnel and into the footing longitudinal opening, with the ligament carrying member to lock therein, completing the ligament graft end mounting in the bone tunnel. With, after the ligament graft is placed under tension and the opposite graft end secured to the tibia cortex, should an adjustment of ligament graft tension be desirable, the footing member is turned appropriately in the bone tunnel.

    摘要: A variable angle drill guide and method for its use for securing an end of a ligament graft in a tunnel section of a straight tunnel section that has been formed into a patient's knee in a procedure for replacing a cruciate ligament. The drill guide includes a straight reference rod that is for fitting to slide through a straight hole formed in a slide block and includes a crossing straight hole that is for receiving, fitted therethrough, a straight base leg rod that is bent from one end into a straight drill guide mounting leg, the mount leg, in turn, to mount an arcuate shaped slide track along its top end. The slide track is curved to approximately one hundred twenty (120) degrees of arc and is arranged to maintain a drill guide slide traveling therealong that includes a drill guide barrel that is fitted to slide therethrough. By adjusting and locking positioning of the reference rod and base leg rod fitted through the slide block, the drill guide barrel will point at a target depression formed in the reference rod, whereby a drill turned through the drill guide barrel will form a passage from any point on the frontal cortex of the distal femur or proximal tibia to the target depression, formed in the reference rod. This formed passage will be at an angle to the tunnel section that is greater or lesser than a right angle and, upon removal of the reference rod and installation of a ligament graft in the straight tunnel, the formed passage can receive a screw turned therein to contact the ligament graft end, for mounting it in the tunnel section.
