Disclosed adjustable retaining arm systems can include a multi-segmented, articulable arm that holds one or more interchangeable tools and can be made relaxed to set a desired position and made rigid to fix the desired position by adjusting tension in a cable running through the arm. The arm can quickly attach and detach from a support structure. The support structure include a cable tensioning system, sensors, control system, power system, etc. A user input device, such as a foot pedal or button on the arm, can be used by a surgeon to change the arm between relaxed and rigid states. The arm and attachable tools can be detachable, disposable and/or sterilizable, while other system components can remain mounted in a fixed location as the arm is removed and replaced. The arm can hold multiple tools in any desired orientation around subject work location.

This disclosure discloses a flexible elongate shaft assembly which includes an elongate flexible tube having at least one joint built into the elongate flexible tube. The joint includes a contact-aided compliant notch topology built into the elongate flexible shaft configured to cause the joint to mechanically interfere with itself and self-reinforce during bending of the elongate flexible shaft resulting in an increase in stiffness of the joint to prevent bucking and plastic deformation of the elongate flexible shaft assembly, and assume a predetermined and designed bending shape of the elongate flexible shaft assembly. The flexible elongate shaft assembly is incorporated into a flexible articulate surgical tool that provides the needed stiffness in order to be able to manipulate tissue and bear loads in anatomically confined spaces. The surgical tool includes a clinician operated handle, the flexible elongated shaft assembly extending from the handle to a surgical tool with the flexible elongate tube having one or more joint sections located near the surgical tool. A flexible cable connects the handle to the surgical tool. The joint sections are configured so that when the clinician activates the surgical tool, a mechanical interference is generated in the joint section and this mechanical interference not only increases stiffness throughout the articulation of the joint's range-of-motion but it also serves the dual purpose of controlling the bent "shape" of the flexible portion of the joint.

An improved gastric tube for displacing a section of an esophagus during cardiac ablation procedures is disclosed. The improved gastric tube is an elongated flexible tube designed to be inserted in the esophagus of a patient and extended past the portion of the esophagus which overlies the heart. The improved gastric tube includes a first lumen extending the length of the tube which receives a control wire, plastic stylet, or other apparatus which would function for displacement of the portion of the esophagus overlying the heart. A second lumen is included which extends to the operative section of the gastric tube, where the esophagus overlies the heart, so that contrast liquid or cooling liquid can be injected into the esophagus at that location. A temperature sensor can also be included to measure the temperature of the esophageal wall, as well as electrodes to connect to a three-dimensional mapping system.

An apparatus comprises a body assembly, a shaft assembly, an end effector, an articulation section, and an articulation control assembly. The end effector is located at a distal end of the shaft assembly and comprises an ultrasonic blade. The articulation section is coupled with the shaft assembly and is operable to articulate to thereby deflect the end effector from the longitudinal axis. The articulation control assembly comprises first and second threaded members and an articulation control. The first and second threaded members have respective first and second pitch orientations. The articulation control is rotatable to thereby drive articulation of the articulation section by causing translation of the first and second threaded members along a path that is parallel to the longitudinal axis of the shaft assembly. The axis of rotation of the articulation control is non-parallel with the longitudinal axis of the shaft assembly.
