摘要:
An endoscopic instrument is described having a fluid chamber, a cannula, a shell, a first locking mechanism, and a probe with a second locking mechanism. The fluid chamber has proximal and distal ends. The hollow cannula is coupled to the distal end of the fluid chamber. The port is located at the proximal end of the fluid chamber. The shell houses the fluid chamber, the port, and the first locking mechanism. The probe extends through the port, through the fluid chamber, and through the hollow cannula. In a first fully inserted position, the distal end of the probe extends past the distal end of the cannula. When the probe is partially withdrawn from the shell, the first locking means releasingly engages the second locking means on the probe. Different spring loaded mechanism are described, including one mechanism which is overcome by pulling on the handle of the probe, and another mechanism which requires an active release mechanism.

An endoscopic electrosurgical suction-irrigation instrument with insertable probes and attachments is disclosed. The instrument has a fluid chamber sealed at its proximal end with a slit valve for receiving the probes and connected at its distal end to a cannula through which the inserted probes extend. The fluid chamber is selectively provided with suction and/or irrigation and includes an electrical contact for supplying voltage to an inserted electrosurgical probe. Depending on the probe configuration suction and/or irrigation is provided in an annular space between the probe and the cannula or through the probe. Valves, connectors and switches for use with the instrument are also disclosed.

A probe for use with an endoscopic suction-irrigation instrument is disclosed which permits a second probe to be inserted therethrough. The suction-irrigation instrument has a cannula, a fluid chamber having at least one port for irrigation and suction and a proximal opening, and a first seal in the proximal end of the fluid chamber. The fluid chamber has a distal end communicating with the cannula. The probe has a proximal handle portion, a middle portion, and a hollow distal portion. The handle portion has an axial passageway and a second seal therein. The middle portion extends from the handle portion through the first seal at the end of the fluid chamber and through the fluid chamber. The middle portion has an outer diameter shaped to contact the first seal such that the proximal end of the fluid chamber is sealed. The middle portion is also shaped to be smaller than the inner diameter of the fluid chamber, and the middle portion is provided with a second axial internal passageway extending therethrough. The distal portion extends at least partially through the cannula and has an outer diameter smaller than the inner diameter of the cannula. A second probe may be inserted through the second seal of the first probe, through the axial passageway in the handle portion, through the internal passageway in the middle portion, and out through the distal portion and the cannula.

An endoscopic suctioning instrument with variable suction is disclosed. The instrument has a hollow cannula having a proximal end and a distal end, and sliding sleeve having a proximal end and a distal end and a longitudinal axis, with the sliding sleeve being coaxial to and closely surrounding and slidingly engaging the hollow cannula. The sliding sleeve has a plurality of lateral holes at its distal end with the lateral holes being spaced along the longitudinal axis. A valve is provided for selectively coupling a suction source to the proximal end of the cannula, and a sleeve actuation mechanism is provided for sliding the sliding sleeve from a first position where the distal end of the sliding sleeve extends beyond the distal end of the cannula such that a first number of the lateral holes are unblocked by said cannula, to a second position where the distal end of the sliding sleeve extends beyond said distal end of the cannula such that fewer lateral holes are unblocked by said cannula, to a third position where the cannula blocks all of said lateral holes of the sliding sleeve. Various embodiments are described with different mechanisms for causing the sleeve to slide, including mechanisms linking the valve to the sleeve.

An endoscopic or laparoscopic biopsy forceps instrument is provided which includes a flexible tubular member having proximal and distal ends, a biopsy jaw assembly at the distal end of the tubular member, an actuation assembly to operate the jaw assembly between open and closed positions, and a control assembly to deflect the biopsy jaw assembly relative to a lumen through which the instrument extends and to rotate the jaw assembly about the longitudinal axis of the instrument.

Releasable ratchet mechanism for medical instruments such as endoscopic instruments are disclosed. The ratchet mechanism includes: a leaf spring with a locking barb which is located on a handle of the medical instrument; a ratchet on a lever arm of the medical instrument which lever arm is pivotally engaged to the handle; and a latch on the handle of the medical instrument. The latch on the handle engages and resiliently deforms the leaf spring to cause engagement of the locking barb in a groove of the ratchet when the latch is in a first position. When in a second position, the latch releases the locking barb from the ratchet groove. Preferably, the locking barb has an extending edge surface on an axis which is substantially parallel to the pivoting axis of the handle and lever arm, and the teeth of the ratchet have extending edge surfaces on axes substantially parallel to the extending edge surface of the locking barb. Also, preferably, the edge surfaces of said teeth of the ratchet are located along an arc of a cylinder having the handle/lever arm pivot axis as its center line.

Disposable laparoscopic surgical instruments for insertion through trocar tubes are disclosed. The instruments broadly include: a hollow aluminum tube; an aluminum clevis which is formed separately from the aluminum tube with the distal end of the hollow aluminum tube crimped around the proximal end of the clevis, and with the clevis including a pivot pin; at least one end effector element having a pivot hole through which the pivot pin of the clevis is pivotally engaged, and another through-hole; an aluminum push rod extending at least partially through the hollow aluminum tube and mechanically coupled to the end effector element; and a plastic actuator engaging the proximal end of the push rod and for imparting reciprocal motion to the push rod relative to the aluminum tube, where the reciprocal motion is translated at the clevis into pivotal motion of the end effector element. Preferably, staple-like connectors are used to mechanically couple the end effector element to the push rod, especially for double acting instruments. The end effector elements are preferably formed of investment cast bronze.
摘要:
Medical instruments are provided having end effectors and end effector actuator apparatus which are rotatable relative to the end effectors. The instruments generally include a tube which is typically coupled to the end effectors via a clevis around which the end effectors pivot, a push rod extending through the tube and coupled to the end effectors, and an actuator apparatus engaging the tube and the push rod with the actuator apparatus including a handle which surrounds and rotatably engages the proximal end of the tube, and a lever arm which pivotally engages the handle and is coupled to the push rod such that pivotal movement of the lever arm relative to the handle effects axial movement of the push rod relative to the tube and pivotal movement of the end effectors. In order to alternatively fix or allow rotation of the actuator apparatus relative to the tube and end effectors, the tube is provided with a plurality of peripherally spaced apart recesses in a portion thereof, and the handle is provided with a resiliently biased contact element which is coupled to an electrode and which when seated in a recess restrains rotation of the outer tube about the longitudinal axis, but which can be unseated by a sufficient relative rotational force. To decouple rotation of the push rod from the lever arm, a generally spherical push rod engaging element is provided in a cylindrical bore of the lever arm.
