A total hip prosthesis comprises a cotyloid implant consisting of a metal cup to be anchored in the acetabulum, a polyethylene insert which provides an intermediate implant inserted into the cavity of the cup, and a femoral implant consisting of a cap having an outer convex surface which is slidably postionable within the insert cavity. An inner concave surface of the femoral implant is implantable on the head of the femur. The prosthesis is particularly suitable for endo-articular implantation.

An osteogenic material packing device is used to pack osteogenic material onto a spinal fusion device. The packing device has a cavity defined therein, which is adapted to receive the spinal fusion device. The packing device further includes an access port, which intersects the cavity to receive the osteogenic material. A cannula is coupled to the packing device and an inserter is coupled to the fusion device in order to insert the fusion device into the packing device. Osteogenic material is packed through the access port around the fusion device. The fusion device then is slid through the cannula and inserted in the intervertebral space between adjacent vertebrae.

A system for stabilizing the vertebral column without requiring the excision of bone from the superior or inferior vertebrae or the resection of the adjacent anterior or posterior longitudinal ligaments. The system includes a stabilizing device and a deployment system composed of first and second coacting tools. The stabilizing device is made up of an expandable threaded insert and an expansion screw. The threaded insert is a hollow cylindrical member having a central cylindrical threaded bore, and a self tapping helical thread extending about the outer periphery of the member. The member is split at its proximal end in communication with the bore to form a pair of slots extending parallel to its longitudinal axis. The expandable insert is arranged to be screwed by the first tool into the intervertebral space between the superior and inferior vertebrae to a desired depth and with the slots oriented in a plane generally perpendicular to the longitudinal axis of the vertebral column. The first tool includes a pair of extending fingers which fit into the slots to achieve that end. When screwed in, the self-tapping threads cut into the cortical bone of the superior and inferior vertebrae contiguous to the intervertebral space, but not substantially into the cancellous bone. The expansion screw is tapered and arranged to be screwed into the internally threaded bore in the expandable insert by the second tool. The second tool fits into the first tool. When the expansion insert is screwed into the expandable insert it causes the slots of the expandable insert to open and thereby spread the superior and inferior vertebrae apart, thereby stabilizing the being's vertebral column.

An improved method and instrumentation for performing spinal surgery, including discectomy, interbody fusion and rigid internal fixation of the spine, from the lateral aspect of the spine is disclosed. The surgical procedure can be performed through a very small incision. The instrumentation of the present invention, all of which is inserted from a lateral position into the spine in the preferred embodiment, comprises a guide pin, a distractor, an extended outer sleeve, an inner sleeve an adjustable drill and an implant driver. The distractor of the present invention is driven into the disc for spacing apart and realigning the adjacent vertebrae. It further functions as an alignment rod for inserting the extended outer sleeve which is a hollow tubular member capable of maintaining said spacing and alignment of two adjacent vertebrae and defines a protected space through which subsequent instruments which may include, but are not limited to, a drill and a diameter reducing inner sleeve may be passed, as well as a spinal implant. The remainder of the surgical procedure consisting of the removal of spinal material across the disc, fusion, and rigid internal stabilization via the implant may all be performed via the closed space within the extended outer sleeve.

Screw-based primary fixation of the prosthetic components within the medullary cavity solves the problem of micro-movements encountered in conventional press-fit cementless fixation. For a total hip prosthesis, the stem is fixed to the medial cortex of the proximal femur by the medial approach alone, obviating the need for drilling of the lateral cortex. The stem may be implanted using special drill guide instrumentation. Anchoring screws are locked into the stem of the femoral component, while self-cutting threads on the screw head engage the pre-drilled medial cortex. This novel fixation principle can also be applied to other joint prostheses, e.g. finger, shoulder, elbow and knee, as well as to dental and spinal implants.

A humeral prosthesis features improved anatomic attachment areas for tendon or bone. In the preferred embodiment, in contrast to existing devices, at least one set of tendon/bone attachment points are provided along a line, at least a portion of which is divergent with respect to the axis of the stem. One or more sets of attachment points may be further be provided along a line which is substantially parallel to the axis of the stem, resulting in a “T” “L” or “U” shape. Alternatively, attachment points having a changing degree of diversion with respect to the axis of the stem may be provided along a common, curved line. The attachment points may simply be apertures formed through the body of the implant though, in the preferred embodiment, the apertures are provided on raised tabs. An area of bone-ingrowth material may be provided adjacent the attachment points, and may include a separate fastening mechanism such as a threaded hole to receive a screw. A groove may also be provided in any embodiment to receive the biceps tendon. Particularly with respect to fractures, including multi- and ‘four-part’ fractures, means specifically intended for the rigid reattachment of the greater or lesser tuberosities may be provided separately or in conjunction with other sets of reattachment configurations.
