摘要: A tissue sampling apparatus comprises a tubular body having a primary lumen for receiving a tissue sample. The tubular body includes a distal end, a proximal end, and a longitudinal axis extending from the proximal end to the distal end. The tissue sampling apparatus further comprises a cutting cylinder having a distal cutting edge, which is movable both distally and proximally relative to the tubular body. A band having an eyelet disposed therein extends across a distal end of the tissue sampling apparatus, wherein the eyelet is advantageously movable relative to the distal cutting edge in order to sever a distal end of the tissue sample.

摘要: An automatic control system for a vacuum-assisted automatic core biopsy device (10) is provided, wherein the automatic core biopsy device (10) comprises a housing (12), out of which extends a needle assembly (14) including a hollow outer piercing needle (16), an inner cutter (18) having a lumen (20), a probe housing (22), and a tissue receiving notch (24). The automatic control system (92) is computerized to move the hollow outer piercing needle (16) automatically to the target tissue lesion, as was true in the prior art, but also is programmed to automatically control the rotational orientation of the piercing needle (16), and its associated tissue receiving notch (24), as well as the axial positioning and rotation of the cutting cannula (18). Consequently, a clinician user need only mark the desired locations within the target location from which tissue samples are desired and the automatic control system will operate the device to retrieve samples from the marked locations.

摘要: This invention provides an inventive tissue sampling probe which offers many advantages over probes available in the prior art. Unexpectedly superior results are obtained in connection with the retrieval of intact tissue specimens, because of a unique combination of cutting features, including, in particular, the employment of an electrosurgical cutting element which is extendible to permit ready severance of the distal end of the tissue specimen, without impact to surrounding tissue. Additionally, the inventive instrument is advantageously designed so that portions of the instrument which contact the patient's tissue or fluid discharge (i.e. blood) during a procedure are modular and disposable, to permit ready replacement of those portions with a new module for an ensuing procedure, without the necessity of cleaning and sterilizing the instrument. The versatility of the invention permits its use in many applications, including, for example, breast biopsies, intraoperative staging, laparoscopic surgery, and lymphadenectomy procedures.

摘要: A medical access device is adapted to receive an instrument and comprises a housing defining an inner channel and a valve disposed in the channel to form a seal with the housing. The valve includes an outer wall and a pair of inner walls extending in distally converging relationship to define a slot. At least one compression member extends between the outer wall and an associated one of the inner walls. The compression member is compressed in response to insertion of the instrument and exerts a force on the associated inner wall when compressed to reduce leakage of the valve when the instrument is inserted. A funnel is provided at the distal end of the valve to facilitate backloading of the instrument.

摘要: A system for creating an arteriovenous (AV) fistula comprises a vessel access sheath having a hollow interior and an exit port, a side access needle catheter configured to fit within the hollow interior of the sheath, a needle configured to be inserted into a blood vessel through the side access needle catheter, a toggle delivery catheter configured to fit within the hollow interior of the sheath, and a toggle apparatus configured to be delivered into a vessel through the toggle delivery catheter. The toggle apparatus comprises a shaft and a toggle member pivotably attached to a distal end of the shaft. A source of RF energy or resistive heat energy may be provided for application to the toggle member and/or to a heater insert in the toggle delivery catheter, for the purpose of creating the fistula.

摘要: A suturing instrument and methods for placing stitches in soft tissues combines the functions of both independently grasping the tissue and passing sutures through the tissue to form a mattress stitch therein. The endoscopic suturing instrument comprises a clamping portion having a longitudinal axis, which is adapted for percutaneous entry into an operative site. The clamping portion further comprises a plurality of jaw members that are movable relative to one another between a closed position wherein the jaw members are disposed in close proximity to one another and an open position wherein the jaw members are spaced apart. A needle portion is disposed at an angle relative to the longitudinal axis, and is adapted for percutaneous entry into said operative site. The needle portion comprises a needle, and preferably a pair of needles, for passage through a portion of soft tissue disposed between the jaw members of the clamping portion.

摘要: A device is provided for stabilizing and positioning an otherwise hand-held medical instrument, such as a biopsy power driver and probe, to ensure the ability to accurately place the probe into the breast and maintain position relative to a particular tissue site from any desired entry point. The device comprises a main arm having a generally vertical portion and a generally horizontal portion. A clamping assembly permits attachment of the apparatus to a stationary object, which is preferably an examination table, and includes a locking clamp having a gripping mode and a releasing mode for alternately gripping and releasing the main arm, and a lever assembly for actuating the locking clamp between the gripping mode and the releasing mode. A horizontal clamp, attached to the generally horizontal portion of the main arm, provides support for an articulating arm assembly having a first arm portion, or upper arm, and a second arm portion, or forearm, wherein the upper arm and forearm are attached together by an elbow joint, so that they may be articulated relative to one another about a common axis disposed through the elbow. A mounting bracket for securing the biopsy power driver and probe is attached to a distal end of the forearm. The elbow joint attaching the upper arm and forearm of the articulating arm assembly comprises a spring loaded counterbalance mechanism which functions to support the weight of the medical instrument, thereby giving the instrument a weightless feel during the positioning process, which prevents fatigue on the part of a user and therefore permits more delicate and precise movements of the instrument.

摘要: A method and device for the automated biopsy and collection of soft tissue having a piercing needle with a receiving port to trap tissue prior to cutting. A motor drive directs and positions the tissue receiving port at a lesion site in arbitrary positions about and along the longitudinal axis of the device. A cutter advances into the receiving chamber and severs tissue which has prolapsed into the receiving port. The severed tissue is then removed from the receiving port without removing the piercing needle receiving port from the lesion site, thus allowing for the accurate, rapid removal of an arbitrary number of core samples with only one insertion. A tissue sample cassette provides storage for the samples as well as a means for coding and decoding the location from which the samples were obtained. Together, these features allow for more accurate and complete sampling of large lesions, for the complete removal of small lesions or removal of other tissue for a variety of reasons.
