    摘要: A bone-fastening system includes an implant with a threaded screw, a distal nut with a threaded bore, and a proximal washer which may also be threaded. An instrument for implanting the implant includes a handle and a barrel terminating in a holder for the washer. A c-shaped bridge assembly has a first end connected to the barrel and a second end including a holder for the nut in axial alignment with the washer. A manually operated control on the instrument, coupled to an elongated element in the barrel, causes the washer to move toward the nut, thereby applying compressive pressure to bone portions to be fastened together. A drive mechanism disposed within the barrel has a distal end with a holder for the screw and a proximal end that is turned by a user, thereby advancing the screw through the washer to the nut to fasten the bone portions under compression. The manually operated control is preferably a trigger that is squeezed toward the handle by a user. A separate control, also preferably in the form of a trigger, is used to release pressure on the elongated element in the barrel, enabling the implant to be easily removed from the implant once in position.

    摘要: Methods, systems, and kits for occipital-cervical spinal fixation are provided. A plate configured for attachment to the occipital bone has two arms extending out from either side, which turn downwards parallel to one another. A first bend is disposed in the arms, such that the arms extend down from the occipital bone behind the spinous process of the C1 and C2 vertebrae, upon installation. A second bend in the arms allows attachment to the C2 vertebra. The system may be dimensioned for pediatric installation. A bone graft material may be held in place between the cervical vertebrae and the skull by installing a cable to the installed system to retain the bone graft material in place.

The system may comprise a U-shaped implantable device having an inferior section, a superior section, a midsection extending therebetween, and pair of lateral walls for engaging a spinous process of a vertebra, each of the lateral walls including an aperture with a countersink for receiving a bone fastener. The system may also include a bone fastener comprising a threaded bolt and a threaded nut for securing the implantable device to the spinous processes.     摘要: A system for providing dynamic stabilization and balance control at a vertebral motion segment has first and second bridge elements and at least one bias element. The bridge elements anchor to adjacent vertebrae with polyaxially adjustable anchoring members, and the bias elements attach to each bridge element to span between them. Each bias element has two fixation portions and a bias body extending between the fixation portions. Each bias element may comprise an elastically deformable material to provide dynamic stabilization with motion, or may comprise rigid material to provide rigid stabilization, and both bias element types may be included in one system. The bias elements are attachable to the bridge elements at discrete attachment locations, or at non-discrete attachment locations. Alternate embodiments may include three or more bridge elements on adjacent vertebrae, and multiple bias elements. A tensioning tool may provide adjustable tension to an elastically deformable bias element.
