摘要:
An improved skin substitute is presented comprised of a silicone layer backed up with a woven nylon fabric layer, the silicone layer possessing a regular pattern of slits that permit the porosity of the skin substitute to be adjusted by clinicians by means of applying tension to the skin substitute that differentially opens the slits. A variety of therapeutic substances can be applied to the skin substitute to promote healing, including aloe and other medicinal preparations. The skin substitute is directed at chronic wound treatment and hernia repair.
摘要:
An improved skin substitute is presented comprised of non-biological materials produced with a series of regularly-spaced pores and a nylon mesh netting. The top component is a thin (approximately 0.001″ thick) silicone elastomer in which pore holes have been vacuum-pulled; physically attached to the silicone elastomer is a fine knitted nylon fabric (12/1, 15/2 denier); incorporated into the silicone/nylon structure are several wound healing agents, including Vitamin E, Vitamin C, Chondroitin 4 Sulfate, Chondroitin 6 Sulfate and Immuno-10 (a highly purified fraction of Aloe Vera) in addition to porcine collagen peptide. The holes in the skin substitute provide a range of porosities to ensure minimum fluid accumulation beneath the skin substitute without wound desiccation. The three-dimensional nature of the nylon mesh in addition to the use of collagen peptides provides better acute adherence with a structure that supports the growth of new skin cells. The range of hole diameters preferred in the present invention is 0.75 mm to 1.05 mm and at holes centered at ¼″-⅓″, providing minimal fluid accumulation beneath the Temporary Skin Substitute, which will reduce infection complications and maximize wound healing.

The Temporary Skin Substitute of this invention consists of three components: The top component is a thin (approximately 0.001″ thick) silicone elastomer in which laser holes have been drilled; physically attached to the silicone elastomer is a fine knitted nylon fabric (12/1, 15/1 denier); incorporated into the silicone/nylon structure are collagen peptides [about 10 micrograms per square centimeter of Porcine type 1—“the active component”] without cross-linking agent to enable a quick interaction with fibrin in the wound to achieve acute adherence. The laser drilled holes provide a wide range of porosity to ensure minimum fluid accumulation beneath the Temporary Skin Substitute without wound desiccation. The range of hole diameters preferred in the present invention is 0.75 mm to 1.05 mm and at holes centered at ¼″-⅓″. Providing a structure that has better acute adherence and minimal fluid accumulation beneath the Temporary Skin Substitute, which will reduce infection complications and maximize wound healing. Larger pieces of this skin substitute can be made to cover larger wounds, unlike previous skin substitutes.

Bio- and blood compatible materials are prepared by treating the surface of a substrate to provide reactive primary or secondary amine groups sites which are activated by treatment with a dialdehyde or arylchloride for coupling to a biological in an amount sufficient to provide compatibility. The use of specific substrates, such as a compliant, and elastic material, such as a fabric-elastomer membrane matrix, results in a product having advantageous qualities as a thermal burn dressing, breast prostheses and implants. Detailed procedures and various products are described.

An improved abdominal hernia repair system is presented comprised of a silicone layer backed up with a knitted or woven polypropylene fabric layer, the silicone layer possessing a regular pattern of slits that permit equilibration of fluid pressure across the device. A variety of therapeutic substances can be applied to the hernia repair device to promote healing, including aloe and other medicinal preparations. A layer of water soluble or water insoluble anti-scar compound is also present, the preferred compound being Salinomycin. An improved inguinal hernia repair system is presented that is identical to the above except it does not contain the hydrophobic silicone component.

A mechanism and a method for slitting silicone membranes using automated slitting blades is presented. The pattern of slits in the membrane can be adjusted by using a cutting cylinder comprised of a plurality of circular cutting gears assembled in parallel. The cutting cylinder is of indefinite length, but in the preferred embodiment is approximately 15″ long and 3″ in diameter. The silicone membrane can range in thickness from 0.005″ to 0.01″ inch.

Detailed procedures and various products are described including gloves and sleeves and tubes with or without adhesive to facilitate formation of sheet materials into various forms and products. The products may have small holes provided therein for the rapid passage of accumulated fluid away from the wound or infusion of liquid materials to the wound site.

An improved dressing is presented which enables quicker wound closure and a lower anesthesia requirement, comprised of an improved prior-art skin-substitute. The improvements consist of variations in the thickness of the woven portion of the dressing, to increase wound adherence.
