Abstract: Systems and methods for detecting worsening cardiac conditions such as worsening heart failure events are described. A system may include sensor circuits to sense physiological signals and signal processors to generate from the physiological signals first and second signal metrics. The system may include a risk stratifier circuit to produce a cardiac risk indication. The system may use at least the first signal metric to generate a primary detection indication, and use at least the second signal metric and the risk indication to generate a secondary detection indication. The risk indication may be used to modulate the second signal metric. A detector circuit may detect the worsening cardiac event using the primary and secondary detection indications.

Abstract: An apparatus comprises plurality of sensors and a processor. Each sensor provides a sensor signal that includes physiological information and at least one sensor is implantable. The processor includes a physiological change event detection module that detects a physiological change event from a sensor signal and produces an indication of occurrence of one or more detected physiological change events, and a heart failure (HF) detection module. The HF detection module determines, using a first rule, whether the detected physiological change event indicative of a change in HF status of a subject, determines whether to override the first rule HF determination using a second rules, and declares whether the change in HF status occurred according to the first and second rules.

Abstract: Devices and methods for improving device therapy such as cardiac resynchronization therapy by determining a value for a device parameter are described. An ambulatory medical device (AMD) can include a sensor circuit to sense a physiological signal and generate two or more signal metrics, and detect an event of worsening cardiac condition using the two or more signal metrics. In response to the detection of worsening cardiac condition, the AMD can determine, for a stimulator, a value of at least one stimulation parameter based on temporal responses of two or more signal metrics. The temporal responses include near-term and long-term responses to the stimulation. The AMD can program the stimulator with the determined parameter value, and generate stimulation according to the determined parameter value to stimulate target tissue.

Abstract: Systems and methods for detecting a target cardiac condition such as events indicative of worsening heart failure are described. A system may include sensor circuits for sensing physiological signals and a signal processor for generating a predictor trend indicative of temporal change of the physiological signal. The predictor trend may be transformed into a sequence of transformed indices using a codebook that includes a plurality of threshold pairs each including onset and reset thresholds. The codebook may be constructed and updated using physiological data. The system may detect target cardiac condition using the transformed indices.

Abstract: Systems and methods for managing machine-generated alerts of a medical event such as worsening heart failure (WHF) are described. A patient management system includes a patient database of information about a correspondence between a plurality of alert thresholds (ATHs) and corresponding clinical outcome indicators (COIs) for a plurality of patients. A control circuit can query the patient database for a matching patient that meets specific query criteria compared to a target patient, retrieve an ATH-COI correspondence associated with the matching patient, and determine the alert threshold using a user input of a COI and the retrieved ATH-COI correspondence. A medical event detector detects the medical event using physiological signals sensed from the target patient and the determined alert threshold.

Abstract: Systems, devices, or methods can be used to detect an event, or series of events, that can indicate worsening of congestive heart failure (CHF), or can be used to identify a subject at an elevated risk for developing CHF. A CHF event predictor can be provided using a characteristic minimum of subject cardiac intervals. In an example, the subject cardiac intervals can be obtained during a night-time period or during periods of reduced subject physical activity. A CHF event predictor can be determined using information about physiologic signals received from a subject, such as from a physiologic sensor associated with an ambulatory or implantable medical device.

Abstract: A system may include a port, at least one sensing circuit, and at least one processor. The port is configured to receive an indication of dosing of medication to treat a pulmonary condition of a heart failure (HF) subject and the at least one sensing circuit configured to sense at least one physiological signal, wherein the physiological signal includes physiological information of the HF subject. The at least one processor includes a parameter module configured to extract values of at least one physiological parameter indicative of health status of the HF subject, and a trending module configured to trend extracted values of the at least one physiological parameter and detect an effect of the dosing of the medication on the HF subject using the trending of the extracted values of the at least one physiological parameter.

Abstract: Systems, devices, and methods for managing alerts associated with a target physiological event such as a worsening heart failure event are described. A system may detect one or more alert onsets using an onset threshold, and one or more corresponding alert terminations using a reset threshold. Alerts may be issued corresponding to the detected alert onsets and alert terminations. The system may compare the alerts to a specified alert characteristic, and iteratively adjust the onset or reset threshold until the alerts corresponding to the adjusted onset or reset threshold satisfy the specified alert characteristic. The adjusted onset and reset thresholds may be presented to a user or a process for detecting the target physiological event.
