    摘要: An endoscopic clip applying system as described which contains a venting system and a clip applying support system which prevents closure of the mechanism before firing. Also, the system ensures proper loading of the clips and prevents them from falling out during loading. Further, the system describes an easily manufacturable, positively opening system which has minimized size requirements. Furthermore, a lock-out mechanism is provided so that the mechanism may not be inadvertently fired. The shaft resists excessive torque as well as holds the clip in place during firing. There are provisions for sealant and non back-up of the clips, as well as features which make the clip properly placed for closure.

    摘要: A surgical device that includes a first jaw having a distal end and a second jaw having a distal end. The second jaw is disposed in opposed correspondence with the first jaw. The first jaw is pivotably coupled to the second jaw. The surgical device also includes a biasing element, such as a spring, that biases the distal end of the first jaw towards the distal end of the second jaw. The device may also include a first driver disposed in the second jaw and coupled to the first jaw. The first driver is configured to cause separation of the first jaw and the second jaw when the first driver is actuated for opening the jaws and to close the first jaw and the second jaw when the first driver is actuated for closing the jaws. The device may also include at least one of a cutting element and a stapling element disposed within the second jaw, preferably a blade rotatably mounted on a wedge. A second driver is configured to move the cutting element and/or the stapling element proximally from a distal end toward the proximal end of the second jaw to at least one of cut and staple a section of tissue disposed between the first and second jaws. By biasing the distal ends of the first and second jaws towards each other, the surgical device may prevent a section of tissue which is disposed between the first and second jaws from escaping out from between the distal ends of the first and second jaws.

    摘要: A carriage assembly for controlling a steering wire steering mechanism within a flexible shaft, for use in gastrointestinal tract surgery, comprises a plurality of selectably engageable motors, including two drive motors for selectably rotating drive shafts of a surgical attachment, two steering motors mechanically communicating with corresponding pulleys around which are coiled directional steering wires communicating with a flexible shaft to enable the selective direction of the flexible shaft within a spatial plane, wherein within the assembly housing the steering motors travel on a steering motor carriage and the pulleys travel on a pulley carriage which is biased by a spring means away from the interior wall of the housing, which biases the steering wires toward a taut state. The bias can be overcome by a carriage motor mechanically communicating with the pulley carriage to cause the flexible shaft to go limp. Also disclosed are a series of surgical attachments which may be coupled to and utilized in conjunction with the carriage assembly.

    摘要: A purse string suturing instrument is provided for quickly and effectively forming a purse string suture in tubular tissue through relatively small cannulas placed in the body. The instrument includes a pair of elongated jaws, each having a row of spaced-apart uniform-size teeth. The jaws are hinged together at one end to clamp the tubular tissue therebetween. The two rows of teeth are offset with respect to each other so that they mesh when the jaws are closed. A single operating mechanism is provided to articulate the pair of jaws with respect to a longitudinal axis of the suturing instrument and actuate the jaws between an open position and a clamping position. The jaws are inserted through a cannula and positioned to close around the tissue to be sutured. The rows of teeth bend the tissue therebetween into a wavelike configuration. A pair of needles, the ends of each being connected to a single length of suture, is then inserted through passageways provided through the teeth. An encircling series of stitches are thus formed in the wall of the tubular tissue to create a purse string suture. The needles and the suturing instrument are then withdrawn through the cannula. The tubular tissue can be snugly fitted over an anastomosis ring or the like by drawing and tying the purse string suture.

The second jaw is mechanically coupled to the first jaw at a proximal end opposite a distal end. A cutting element is disposed within the second jaw, and a first driver is configured to move the cutting element proximally from the distal end toward the proximal end of the second jaw to cut a section of tissue disposed between the first and second jaws. The device may also include a stapling element disposed within the second jaw. The cutting element and the stapling element may be contiguous so as to define a cutting and stapling element, such as a wedge having a blade disposed thereon. As the wedge is moved proximally from the distal end of the second jaw to the proximal end, the wedge pushes a plurality of staples against a plurality of opposing staple guides disposed in the first jaw in order to staple a section of tissue while cutting the section of tissue.
