A system for detecting whether a vascular access has been interrupted in an arrangement in which two catheters or needles are present in a blood vessel, fistula or graft. A fluid line leading to a pump is connected via a first connector to a first indwelling catheter, and a fluid line leading from a pump is connected via a second connector to a second indwelling catheter. Each connector is equipped with an electrode in contact with the lumen of the connector, the electrodes electrically connected to an electronic circuit that measures the impedance or conductivity of fluid between the first connector and second connectors via a fluid path through the blood vessel, fistula or graft. An electronic controller receives the impedance or conductivity data and processes the data to determine whether a vascular access disconnection has occurred. The processing may involve filtering the signal received by the controller, and/or setting provisional flags for a disconnection event that may be cleared if the signal changes before the expiration of a counter.

A system, method and apparatus for performing a renal replacement therapy is provided. In one embodiment, two small high flux dialyzers (20, 30) are connected in series. A restriction (40) is placed between the two dialyzers in the dialysate flow path (962). The restriction is variable and adjustable in one preferred embodiment. The restriction builds a positive pressure in the venous dialyzer, causing a high degree of intentional backfiltration. That backfiltration causes a significant flow of dialysate through the high flux venous membrane directly into the patient's blood. That backfiltered solution is subsequently ultrafiltered from the patient from the arterial dialyzer. The diffusion of dialysate into the venous filter and removal of dialysate from the arterial dialyzer causes a convective transport of toxins from the patient. Additionally, the dialysate that does not diffuse directly into the patient but instead flows across the membranes of both dialyzers provides a diffusive clearance of waste products.

The system identifies the particular dialyzer (16) to be cleaned, has the capability of providing different cleaning processes for different types of dialyzers, for performing various reuseability tests to determine whether the cleaning process is proceeding properly and has the ability to inhibit the further sequencing or to add additional cleaning sequences if the various tests are not passed. The system insures that a dialyzer is properly cleaned and is returned for use only with the proper patient.
