摘要: A multi-seal trocar system (10) includes a plurality of co-axial sealing elements (30 60,80) adapted for forming a seal with either a large or small instrument. Small and large scaling elements (45,62) are disposed in an in-line arrangement such that a single port is provided for instrument insertion. A zero seal (80) is disposed distally of the sealing elements to seal a working channel of the system and to prevent backflow when no instruments are inserted The small sealing element (45) is configured such that insertion of a large instrument automatically moves the small sealing element (45) out of its path, thereby avoiding contact with the large instrument

摘要: A surgical trocar includes an operative sleeve adapted for disposition across a tissue barrier and an obturator removably disposed in the sleeve. An obturator includes a cutting element which is operable to cut the tissue. The obturator and the operative sleeve can be advanced through the cut tissue and the obturator removed leaving the sleeve operatively disposed for further surgery. A cutoff and reset mechanism is responsive to a condition present when the obturator penetrates the tissue barrier to inhibit further cutting by the cutting element. A reset mechanism is manually operable through the conscious efforts of the surgeon to reset the trocar should further cutting be desired. The cutoff and reset mechanisms can be embodied mechanically as well as electrically and adapted to both mechanical and electrosurgical trocars.

摘要: A retraction device for retracting an incision in a body wall includes a sheath that traverses through the incision from outside the body to a body cavity inside the body. The retraction device may include a deployable, adjustable frame that supports the sheath and that, together with the sheath, retracts the incision. The retraction device may include an outer ring and an inner ring coupled to opposing ends of the sheath and further include means to place the sheath in tension between the inner and outer rings, thereby retracting the incision.

摘要: The invention is directed to a trocar adapted to form a seal around a surgical instrument, the trocar comprising a cannula having an axis extending between a proximal end and a distal end; a housing having a proximal wall and forming with the cannula a working channel sized and configured to receive the instrument; a septum seal disposed in the housing and extending transverse to the axis of the cannula across the working channel, the septum seal having a normal state when the instrument is absent from the working channel and a stretched state when the instrument is being withdrawn from the working channel; portions of the proximal wall of the housing defining an orifice sized and configured to receive the instrument into the working channel; and an elastomeric ring-shaped projection extending axially distally between the proximal wall and the septum seal, the ring-shaped projection having an axial length adapted to deform to prevent inversion and binding of the septum seal and the orifice upon withdrawal of the surgical instrument. In particular, the septum seal is prevented from reaching the orifice in the proximal wall of the housing when the instrument is withdrawn. The projection may be bonded, insert-molded, or compressively fitted to the proximal wall of the housing. The projection may be formed of an elastomeric material such as natural or synthetic rubber. The projection may further comprise a plurality of portions or fingers extending axially distally from the proximal wall, each of the extending portions or fingers may further include a living hinge. In another aspect, the ring-shaped projection may be formed on the surface of the septum seal instead of the proximal wall of the housing. That is, the ring-shaped projection may be formed to extend axially proximally from the surface of the septum seal toward the proximal wall of the housing.

摘要: A medical device for viewing inside a body and for retrieving an object from a location within the body, the medical device comprising an endoscope having a tube defining a working channel and a basket engageable with the object. In one embodiment of the invention, the basket does not require a sheath and is disposed through the working channel such that the endoscope operates to contain or activate the sheathless basket. In another embodiment of the invention, a single push wire with a collar at its end is used in place of a sheath to control the opening and closing of the basket. In another embodiment of the invention, a tapered and funnel-shaped sheath surrounding the basket is used to contain or activate the basket. The invention minimizes the basket crossing profile so as to improve fluid flow in the working channel.

摘要: A surgical access device, such as a trocar, includes a pendent valve having an elongate structure extending from a proximal end to a septum valve disposed at a distal end. In operation, the elongate structure follows the angle of the instrument to pre-position the septum valve into the path of the instrument where it is not significantly challenged during instrument insertion or manipulation. The pendant valve can be made to float at both the proximal end and the distal end of the elongate structure, to further reduce the vulnerability of the septum valve. Since the valve is less vulnerable to instrument insertion, it can be formed to minimize friction and maximize the functional range of the access device.

摘要: The invention is directed to nanostructure surface treatments, coatings or modifications formed from nanoscale building blocks. The nanostructure surface treatments, modifications or coatings have hydrophobic, hydrophilic and surface adherence properties. The nanoscale building blocks have orientation, geometry, packing density and composition that may be adjusted to control the unique surface characteristics of the desired treatment, coating or modification. Applications of this nanostructure technology include surgical clips, staples, retractors, sutures and manipulators where an improvement in traction, retention or occlusion is desired without excessive material or tissue deformation or where high compressive forces would be undesirable, dangerous or ineffective. In one aspect, a nanostructure surface treatment for a medical device having an external surface is disclosed, wherein the treatment is applied on the external surface to provide a hydrophobic or a hydrophilic surface. With this aspect, the treatment comprises titanium dioxide and provides nanoscopic structures having nearly vertical sidewalls. The treated surface of the device has contact angles greater than or equal to 150 degrees. The vertical sidewalls provide a negative capillary effect and have a width of about 200 nm. The vertical sidewalls attach to a wet surface by the negative capillary effect. The van der Waals forces of the vertical sidewalls enable the treated surface to attach to a dry surface. The treatment may be vapor deposited and cured on the device, or the treatment may be laser blasted on the device.

摘要: A surgical clamp includes a shaft having clamping jaws at a proximal end and an operative handle assembly at a distal end. The shaft has properties for being moved to a desired shape by the user prior to operation of the handle assembly and for holding the desired shape during operation of the handle assembly. The shaft may include a bendable element extending along an axis between the handle assembly and the jaws. A control element disposed relative to the bendable element is operable from the handle assembly at the proximal end of the shaft to move the jaws relative to each other at the distal end of the shaft. An associated method includes the step of bending the shaft to a desired shape prior to operation of the clamp and maintaining the shaft in substantially the desired shape during operation of the clamp.

摘要: A suturing apparatus adapted to close an incision and an anterior tissue wall includes an elongate support structure having an axis extending between a proximal end and distal end. A receiver disposed at the distal end is insertable through the incision in a low-profile state and expanded to a high-profile state on the far side of the tissue wall. Needles carried by the support structure are operable to extend through the support wall on either side of the incision and into the expanded receiver. Sutures advanced through the needles are captured by the receiver after the needles are removed and the receiver is returned to its low-profile state. Removal of the suturing apparatus from the incision draws the captured suture ends outwardly through the incision. A novel suture knot can be tied in the four suture ends and moved into proximity with the tissue wall to close the incision.
