摘要： Patient respiration may be characterized using a marked respiration waveform involving a respiration waveform annotated with symbols, markers or other indicators representing one or more respiration characteristics. A respiration waveform may be acquired by sensing a physiological parameter modulated by respiration. A marked respiration waveform may be generated based on the acquired respiration waveform and one or more detected respiration waveform characteristics and/or respiration-related conditions. One or more components used to generate the marked respiratory waveform may be fully or partially implantable.

Disordered breathing therapy may be delivered in response to prediction or detection of disordered breathing events. Various conditions affecting the patient may be evaluated and the baroreflex therapy modified. The therapy may be modified to improve therapy efficacy, to reduce an impact to the patient and/or to mitigate therapy interactions.

At least one of the implantable device and the patient-external respiratory therapy devices includes a posture detector. Posture information is transferred between the implantable device and the patient-external respiratory therapy device. The posture information may be used in connection with sleep detection or to modify therapy delivered by the implantable cardiac device and/or the patient-external respiratory therapy device.

The plurality of disordered breathing devices includes at least a therapy device that delivers an electrical stimulation therapy modulating a patient's baroreflex response. Other therapy devices may include a cardiac electrical stimulation device, an external respiratory therapy device, and/or other therapy devices used in the treatment of disordered breathing. A therapy controller coordinates the therapies delivered by the plurality of therapy devices.

摘要： A system and method for determining pulmonary performance from transthoracic impedance measures is provided. Transthoracic impedance measures collected by an implantable medical device are correlated to pulmonary functional measures. The pulmonary functional measures are grouped by respiratory pattern. Pulmonary performance is evaluated. Differences are determined by comparing the pulmonary functional measures for each respiratory pattern to the pulmonary functional measures for at least one previous respiratory pattern. A trend is identified from the differences. An alert is generated upon sufficient deviation of the trend from a threshold criteria.

摘要： Devices and methods for detecting disordered breathing involve determining that the patient is asleep and sensing one or more signals associated with disordered breathing indicative of sleep-disordered breathing while the patient is asleep. Sleep-disordered breathing is detected using the sensed signals associated with disordered breathing. The sensed signals associated with disordered breathing may also be used to acquire a respiration pattern of one or more respiration cycles. Characteristics of the respiration pattern are determined. The respiration pattern is classified as a disordered breathing episode based on the characteristics of the respiration pattern. One or more processes involved in the detection of disordered breathing are performed using an implantable device.

摘要： An implantable respiration monitor can be used to detect disordered breathing or periodic breathing events that can be categorized, such as according to one or more of sleep, exercise, and resting awake states. The categorized frequency of such events can be compared to independently specifiable thresholds, such as to trigger an alert or responsive therapy, or to display one or more trends. The information can also be combined with detection of one or more other congestive heart failure (CHF) symptoms to generate a CHF status indicator or to trigger an alarm or responsive therapy or to display one or more trends. The alert can notify the patient or a caregiver, such as via remote monitoring. The sleep state information can be further categorized according to central sleep apnea (CSA) or obstructive sleep apnea (OSA) events.
