    摘要: Radioactive therapeutic seeds include a carrier structure bearing a radioactive isotope and a radiopaque marker. According to several embodiments, the seeds include a central plug provided with an axial marker and a relatively transverse bore extending through the marker. A second marker may be positioned in the bore, thereby distinguishing a seed provided with the second marker relative to a seed not provided with the second marker. According to another embodiment, the isotope is deposited on the outer surface of a hollow radiolucent tube. A biologically-compatible, radiolucent, surface-sealing layer seals the external surface of the tube. A radiopaque marker wire of selected length is positioned in the hollow of the tube. Seeds may be distinguished from one another by providing seeds with marker wires of different lengths. According to other embodiments, the seed includes a cylindrical outer member, and an inner member provided within the outer member and having two large diameter portions and a relatively smaller diameter portion therebetween. A marker is disposed within the inner member, and a radioactive isotope is carried on the inner member. The marker may be drilled to a smaller size. Each embodiment permits at least two groups of seeds to be distinguished from one another by the use of differing marker configurations.

    摘要: Endoscopic bipolar scissor blades are formed in a laminate assembly of an electrically conductive electrode, an electrically insulating material, and a metal shearing surface which is electrically insulated and physically spaced from the electrode by the insulating material which is disposed therebetween. In one embodiment, the electrode is a metal blade, while the insulating material is a ceramic which is fixed to the metal blade, and the metal shearing surface which also provides the cutting edge is of a metal material which is fixed to the ceramic. In a second embodiment, the insulating material is a molded ceramic blade, with the electrode and the metal shearing surface layer metalized or otherwise fixed to the ceramic blade. In a third embodiment, the electrode is metalized on a thin layer insulating material which is fixed onto a metallic blade. In a fourth embodiment, metallic shearing and electrode layers are laminated to opposite sides of a fiberglass material. In all embodiments, the metal cutting edges and surfaces are insulated from the electrodes, and no short circuit can form between the electrodes even though the cutting edges and shearing surfaces are metal.

    摘要: An endoscopic bipolar multiple sample bioptome includes a relatively long conductive flexible member having a lumen with an axially displaceable conductive wire extending therethrough and covered with an electrically insulating sheath. The proximal ends of the flexible member and wire are coupled to a manual actuation device for axially displacing one of the flexible member and wire relative to the other and the manual actuation device is provided with bipolar electrical couplings for electrically coupling one pole of a current source to the flexible member and the other pole of the current source to the wire. The distal end of the flexible member is mechanically and electrically coupled to a partially conductive cylinder having a distal edge and the distal end of the wire is mechanically and electrically coupled to the jaw assembly. The jaw assembly includes a pair of opposed toothed jaw cups each of which is coupled by a narrow resilient arm to a base member. The base member is mounted inside the cylinder and axial movement of the base member relative to the cylinder draws the necks of the jaws into the cylinder to bring the jaw cups together in a biting action. The cylinder is substantially non-conductive with portions of its exterior being conductive. Cautery current flows between the jaw assembly and the conductive portion(s) of the cylinder by way of tissue which is being cauterized.

    摘要: An endoscopic irrigation instrument includes a fluid chamber, a cannula coupled to a distal end of the fluid chamber, a tapered irrigation tube which couples at its proximal end to a fluid source and which couples at its distal end to an irrigation port of the fluid chamber, and a pinch valve which controls the flow of fluid from the irrigation source to the fluid chamber and hence through the cannula to the surgical site. The tapered irrigation tube is arranged so that it has a first relatively larger constant inner diameter and relatively larger wall thickness on its proximal end until it tapers down in both inner diameter and wall thickness at a location slightly proximal the pinch valve. By the time the tapered irrigation tube passes the pinch valve in the endoscopic instrument, the tapered irrigation tube is at a second relatively smaller constant inner diameter and a relatively smaller wall thickness. The tapered irrigation tube thus enables greater fluid flow, pinch valve control at the thin distal end, and tube reliability at both high and low fluid pressures.

    摘要: An apparatus for counting the number of times a temperature cycle occurs includes an indicator having sequential indicia and a temperature responsive member for indicating a next one of the indicia. Mechanical, fluid mechanical and electronic versions of the invention are disclosed. In the mechanical version, the indicator can be a ratchet member having teeth, a pawl and a display surface containing indicia. A temperature responsive element such as a bimetallic member engages the teeth of the ratchet and advances the ratchet each time the apparatus is subjected to a preset temperature cycle. A housing with a window masks the display to indicate one of the indicia as advanced by the temperature responsive element. Preferred embodiments of the mechanical version include a ring-like ratchet member with interior teeth and an exterior display surface. The number of teeth is preferably one or more less than the number of indicia so that upon advancing the ratchet member to the last indicia, it can be advanced no further. The bimetallic element is mounted inside the ring and the housing covers the outer surface of the ring. A removable locking pin is provided to prevent movement of the ratchet member until activated.

    摘要: An endoscopic instrument with suction/irrigation end effectors includes a tube, a push rod which extends through the tube, an actuator engaging the tube and the push rod, and end effectors coupled to the tube and push rod, where the end effectors include or define a bore which acts as a lumen for suction/irrigation. A fluid conduit in the tube communicates with the bore in the end effectors and is coupled to a suction or irrigation source. In one embodiment, a pair of end effectors are each coupled to a separate irrigation/suction conduit which is fed through the tube. The proximal ends of the conduits are coupled to one or separate fluid couplings at the proximal end of the instrument for connection with suction/irrigation sources. In another embodiment, a stationary end effector is provided with a fluid bore which is coupled to a fluid conduit integral with the tube. In both of these embodiments, the fluid bore(s) terminate(s) in one or more openings on a surface of the end effector(s) so that suction/irrigation is provided on a surface of the end effector(s). In a third embodiment, the annulus between the push rod and the tube acts as a fluid conduit for suction/irrigation and each end effector is provided with a longitudinal surface groove which defines a lumen therebetween so that suction/irrigation may be performed when the end effectors are in the closed position.

    摘要: Hook scissors end effectors are provided for a laparoscopic instrument. The hook scissors end effectors generally comprise first and second blades, with at least one of the blades pivotally engaging the laparoscopic instrument. Each blade comprises an elongate straight blade member with an integral transverse hook element at the distal end of the blade member. The hook elements each extend towards the other blade member and each hook element is offset from its blade member so that a narrow recess is provided between opposing first and second hook members upon closing of the first and second blade members. The elongate straight blade members provide a straight scissors cutting action therebetween so that the contiguously adjacent portions of the blade members gradually come together in a continuous bearing contact to effect a cutting action. While the hook elements at the distal ends of the blades effectively provide a hook which contains the object to be severed, the distal portions of the blade members do not establish contact with each other until the cutting action reaches the distal portion of the scissors.
