摘要: Devices, systems, or methods include generating, by stimulation generation circuitry, a first train of electrical stimulation pulses at a first frequency to a first target tissue; and generating, by the stimulation generation circuitry, a second train of electrical stimulation pulses at a second frequency to a second target tissue different from the first target tissue, wherein at least some electrical stimulation pulses of the first train of electrical stimulation pulses are interleaved with at least some electrical stimulation pulses of the second tram of electrical stimulation pulses. At least the first target tissue is associated with a spinal cord of a patient, and the second target tissue is associated with at least one of a peripheral nerve or a nerve root of the patient.

摘要: Digital-to-Analog (DAC) circuitry for an implantable pulse generator is disclosed which is used to program currents at the electrodes. Calibration circuitry allows the positive and negative currents produced at each electrode to be independently calibrated to achieve an ideal (linear) response across a range of amplitude values provided to the DAC circuitry by a digital amplitude bus. The calibration circuitry includes electrode gain and electrode offset circuitry for each of the electrodes. Current range DAC circuitry is also provided which can be used to adjust the gain and offset current at all of the electrodes. The current range DAC circuitry is particularly useful when spanning a range of therapeutic currents for a patient, and allows all possible amplitude values provided by the digital bus to be used to span the range. This can improve (reduce) the current resolution of the electrode currents with each amplitude value step.

摘要: This document discusses, among other things, systems and methods for managing pain of a subject. A system includes a first sensor circuit to sense a first signal indicative of a functional state of the subject, a second sensor circuit to sense a second signal different from the first signal, and a controller circuit. The controller circuit may determine an operating mode of the second sensor circuit according to the sensed first signal, trigger the second senor circuit to sense the second signal under the determined operating mode, and generate a pain score using at least the second signal sensed under the determined operating mode. The pain score may be output to a patient or used for closed-loop control of a pain therapy.

摘要: Provided is an intraesophageal electrostimulator configured so that other medical tools can also be easily inserted into an esophagus and misalignment in the esophagus can be reduced. An intraesophageal electrostimulator 100 includes a stimulator body 101, stimulating electrodes 111, 112, and power feed lines 113, 114. The stimulator body 101 is formed, in order to insert the stimulator body 101 into an esophagus S, in an elongated flat plate shape with flexibility in a longitudinal direction. The stimulator body 101 includes a first flat plate member 102, a second flat plate member 103, and an exterior body 104. On one plate surface of the first flat plate member 102, each of the stimulating electrodes 111, 112 is provided so as to protrude from the plate surface. The second flat plate member 103 overlaps with the other plate surface of the first flat plate member 102 such that the second flat plate member 103 and the first flat plate member 102 sandwich the power feed lines 113, 114. The exterior body 104 covers the first flat plate member 102 and the second flat plate member 103 overlapping with each other.

摘要: Methods and systems for testing and treating spinal cord stimulation (SCS) patients are disclosed. Patients are eventually treated with sub-perception (paresthesia free) therapy. However, supra-perception stimulation is used during “sweet spot searching” during which active electrodes are selected for the patient. This allows sweet spot searching to occur much more quickly and without the need to wash in the various electrode combinations that are tried. After selecting electrodes using supra-perception therapy, therapy is titrated to sub-perception levels using the selected electrodes. Such sub-perception therapy has been investigated using pulses at or below 10 kHz, and it has been determined that a statistically significant correlation exists between pulse width (PW) and frequency (F) in this frequency range at which SCS patients experience significant reduction in symptoms such as back pain. Beneficially, sub-perception stimulation at such low frequencies significantly lowers power consumption in the patient's neurostimulator.

摘要: New waveforms for use in an implantable pulse generator or external trial stimulator are disclosed which mimic actively-driven biphasic pulses, and which are particularly useful for issuing low frequencies pulses. The waveforms comprise at each electrode interleaved first and second pulses. Each first pulse comprises a first monophasic pulse and a first passive charge recovery period. Each second pulse comprises a second monophasic pulse with a polarity opposite the first monophasic pulse and a second passive charge recovery period. Preferably, the amplitudes and pulse widths of the first and second monophasic pulses are equal, or at least charge balanced at each electrode. The first and second monophasic pulses mimic the functionality of a symmetric biphasic pulse, with the first monophasic pulse mimicking the functionality of the biphasic pulse's first phase, and the with the second monophasic pulse mimicking the functionality of the biphasic pulse's second phase.

摘要: An example method for electrical stimulation of a subject includes creating multiple circuits using implantable electrodes positioned in the subject, transmitting a signal of a first frequency through a first circuit of the multiple circuits and the first circuit generates a first electrical field, and transmitting a signal of a second frequency through a second circuit of the multiple circuits and the second circuit generates a second electrical field. The implantable electrodes are positioned in a substantially linear configuration along a same axis such that the first electrical field and the second electrical field are in an axial bias configuration, and the first electrical field and the second electrical field interfere with each other at an area of overlap to produce a beat signal.

摘要: Methods and systems for recording evoked potentials evoked during electrical stimulation of a patient's neural tissue are disclosed. The methods and systems are useful with treatment modalities, such as spinal cord stimulation (SCS). The disclosed methods and system allow for stimulation artifacts to be reduced in, or removed from, the recorded signals. A residual portion of the stimulation artifact can be modeled and subtracted from a recorded signal that includes both artifact and neural response contributions.
