This document describes, among other things, systems, devices, and methods that use frequency domain analysis of a thoracic signal. One example uses frequency domain analysis for discriminating between different pulmonary physiological states. Examples of breathing states include normal breathing, periodic breathing, Cheyne-Stokes breathing, obstructed respiration, restrictive respiration, and pulmonary edema. The frequency domain analysis may also be used for performing heart rate variability (HRV) diagnostics. In one example, a frequency domain adaptive filter implements a variable cutoff frequency for separating heart contraction spectral content and other spectral content from lower frequency respiration spectral content and other lower frequency spectral content.

This patent document discusses, among other things, systems, devices, and methods for predicting an occurrence of impending thoracic fluid accumulation and in one example, invoking a responsive therapy, such as to prevent or minimize the consequences of the impending thoracic fluid accumulation. One example of the present systems, devices, and methods senses or receives at least one parameter that is statistically associated with impending thoracic fluid accumulation from a subject. Using such parameter(s), a probability of impending thoracic fluid accumulation is estimated. A list of parameters determines which values are recurrently sensed or received at various desired time intervals. Another example of the present systems, devices, and methods weights the sensed or received parameter value(s) to compute the probability estimate of impending thoracic fluid accumulation. A responsive preventive thoracic fluid accumulation therapy or other therapy is selected and activated using the probability estimate of impending thoracic fluid accumulation.

The detection or prediction of an event affecting the respiration of a patient initiates acquisition of information associated with the event. The respiratory logbook system acquires information associated with the event during the event and during intervals proximate in time to the event. The information is organized as a respiratory log entry. The user can access the information by operating a user interface. The information may be presented in textual or graphical form.

Conditions sensed via sensors of an external respiratory therapy device are used to assess a patient's cardiopulmonary status. The respiratory therapy device sensors may be utilized alone or in combination with other sensors to determine cardiopulmonary status of a patient. Therapy delivered to the patient is controlled based on the cardiopulmonary status assessment. For example, therapy delivered to the patient may be initiated, terminated, and/or modified based on the assessed cardiopulmonary status of the patient. Cardiopulmonary status assessment, therapy control, or both, are performed by an implantable device.

Disordered breathing events may be classified as central, obstructive or a combination of central an obstructive in origin based on patient motion associated with respiratory effort. Central disordered breathing is associated with disrupted respiration with reduced respiratory effort. Obstructive disordered breathing is associated with disrupted respiration accompanied by respiratory effort. A disordered breathing classification system includes a disordered breathing detector and a respiratory effort motion sensor. Components of the disordered breathing classification system may be fully or partially implantable.
