An endoscopic biopsy forceps has a detachable proximal handle portion and a distal portion which includes a pair of jaws mounted at the distal end of a flexible conduit. At least one pull wire extends through the conduit and is coupled to the jaws so that movement of the pull wire(s) through the conduit causes the jaws to open and close. The proximal end of the pull wire(s) is provided with a mating tip and the proximal end of the conduit is provided with a mating sleeve. The handle portion includes a shaft and a spool. The conduit is detachably attached to the shaft, while the pull wire(s) is detachably attached to the spool. Attachment is effected by spring biased coupling devices which engage the mating tip of the pull wire(s) and the mating sleeve of the conduit.

An endoscopic bipolar cautery biopsy forceps device has a conduit, a pair of articulable opposed jaws, two conductive pull wires, and an actuator. The articulable opposed jaws are located at the distal end of the conduit. Each jaw is provided with a cutting surface at its distal end and a tang at its proximal end. At least a portion of the cutting surface is electrically conductive and a portion of the tang is electrically conductive and electrically coupled to the conductive cutting surface. The remainder of the jaw is preferably non-conductive or is coated with an insulator. The conductive pull wire is coupled to the conductive portion of the tang of each jaw and is insulated as it extends through the conduit to a handle portion which includes the actuator. The actuator is coupled to the proximal ends of the pull wires so that movement of the actuator pulls the pull wires and articulates the jaws. The actuator is also provided with an electrical coupling device which electrically couples to the proximal ends of the pull wires and permits easy connection to an electrical cautery source.

Remote control actuators for endoscopic tools are provided. Each remote control actuator includes a driver, a coupler which couple the endoscopic tool to the driver, a power source which power the driver, and a controller coupled to the power source and/or the driver for controlling the power applied to the driver. In conjunction with the controller, a user interface is preferably provided for user control of the remote control actuator which actuates the endoscopic tool. In addition, a sensing/feedback mechanism is preferably provided between the driver and the endoscopic tool and is coupled to the controller and/or the power source. The sensing/feedback mechanism senses the amount of tension or compression on the pull wire(s) of the endoscopic tool and provides a related signal to the controller so that the force applied to the endoscopic tool may be regulated. Various types of power sources are disclosed including electric, mechanical, hydraulic, and pneumatic. Various types of drivers are disclosed, including a motor/linear actuator, a hydraulic or pneumatic piston, and a motor with or without reduction gears coupled to a pulley and belt or a sprocket and chain or a rack and pinion. In addition, several couplers, controllers, user interfaces, and sensing/feedback mechanisms are disclosed.

Endoscopic biopsy forceps jaws having an outer edge which may be generally U-shaped, circular, or oblate circular are disclosed with teeth which extend completely around the outer edge. The teeth may take any of several arrangements. In a first arrangement, the teeth and valleys are formed with two substantially rectangular surfaces which meet at substantially horizontal vertex and nadir lines. In a second arrangement, each tooth has a vertex or peak located upward and inward from the outer edge, and is defined by a substantially triangular inner surface, a substantially quadrilateral base, and a surface which extends outward and downward from the vertex of the triangular surface to the outer edge of the jaw. Valleys between the teeth have two inclined surfaces which intersect along a line which extends inward and downward from the outer edge of the jaw. In a third arrangement, the peak of each tooth is defined by the vertex of a pyramid having four faces and valleys between these teeth have three substantially flat surfaces which intersect at a vertex which lies downward and inward from the outer edge of the jaw. Alternate embodiments of all arrangements include providing a lip around the outer portion of the outer edge so that the teeth and valleys are spaced farther in from the outer surface of the jaw cup.

An end effector assembly for an endoscopic instrument includes first and second end effectors, a screw and optionally a washer. The end effectors have arms which are biased away from each other. The screw has a threaded portion dimensioned to engage the distal end of the endoscopic instrument and a head portion with a pair of arm receiving grooves. In a presently preferred embodiment, the washer extends over arms of the end effectors which pass through grooves in the head of the screw. In this manner, the arms of the end effectors are captured between the screw and the washer. The grooves are preferably provided with upstanding pins which engage mounting holes in the arms of the end effectors.

An endoscopic multiple sample bioptome includes a hollow outer member and an axially displaceable inner member extending therethrough. The proximal ends of the outer and inner members are coupled to an actuator for axially displacing one relative to the other. The distal end of the outer member is coupled to one of a cylinder having a sharp distal edge and a jaw assembly, while the distal end of the inner member is coupled to the other. The jaw assembly includes a pair of opposed, preferably toothed jaw cups each of which is coupled by a resilient arm to a base member. The arms are bent to lie in planes which intersect the axis of the base member and to locate the jaws apart from each other. The base member is mounted inside the cylinder and axial movement of the jaw assembly and cylinder relative to each other draws the arms into the cylinder and brings the jaw cups together in a biting action. The biting action is enhanced according to the invention by arranging the cutting edge of each jaw to lie in a plane which intersects the plane in which the distal portion of the jaw arm lies. The biting action is further enhanced by providing a closing cam on the outer surface of the jaws which is engaged by the cylinder when the jaws are closed. The arrangement of the cutting edges also presents a smooth surface to the interior of an endoscope and the closing cams prevent the sleeve from sliding completely over and off of the jaws.

A non-metallic end effector for use in an endoscopic surgical tool includes a metallic core for strength and for providing a selected electrode surface on the end effector. Selectively conductive end effectors are manufactured by insert molding a plastic or ceramic or other non-conductive body around a metallic or otherwise conductive core. The conductive core is exposed on a selected portion of the working surface of the end effector and extends through the non-conductive body of the end effector for coupling to an electrical source. Selectively conductive end effectors of this type can also be manufactured by coating a cast end effector member with a non-conductive polymer or by metallically plating a non-conductive ceramic end effector member. By extending the conductive core of a selectively conductive end effector member to the pivot hole of the member, electrical coupling can be made through the clevis and tube of an endoscopic instrument. By extending the conductive core to the actuation bore of an end effector member, electrical coupling can be made through the push rod or other actuation member in an endoscopic instrument. Using both techniques, a bipolar selective cautery end effector can be provided on an endoscopic instrument.

When the handle assembly is actuated, the push/pull wire moves the disk segment and hence the cylindrical member relative to the end effectors and causes the cylindrical member to ride over the arms of the end effectors, thereby forcing the end effectors closed or permitting them to open. Preferably, the disk segment has two curved sides which are coupled to the cylindrical member by crimping, and two substantially parallel sides which provide room for the arms of the end effectors so that the disk segment can move relative to the end effectors. The disk segment also includes a central bore for receiving the push/pull wire.
