    摘要: A method for cardioverting or defibrillating a human heart, performed by placing a transvenous lead having a cardioversion or defibrillation electrode such that the electrode is located at least partially within the middle cardiac vein of a patient's heart and placing an additional cardioversion or defibrillation electrode in the superior vena cava of the patient's heart and thereafter delivering a cardioversion or defibrillation pulse between the first and second electrodes. The method may be practiced by placing the transvenous such that the defibrillation electrode it carries extends around the apex of the patient's heart, and the transvenous lead may be advanced through one cardiac vein toward the apex of the patient's heart and thereafter advanced upward through a different cardiac vein.

    摘要: A holding member for an implantable cardiac device facilitates tether attachment and removal, at time of implant, and snaring, or otherwise capturing, for subsequent explant. Preferably located in proximity to a proximal end wall of a shell of the device, the holding member includes a strut portion, being spaced proximally apart from the proximal end wall, a waist portion, defining a recess, and an engagement section, extending between the strut portion and the waist portion and overhanging the recess. The waist portion may either extend between the strut portion and the proximal end wall, or be formed in the shell, distal to the proximal end wall. Alternately, the holding member includes a loop element and an engagement element coupled thereto, between first and second segments thereof. The segments are initially formed to give the loop element an opening, and have a flexibility to be compressed together.

    摘要: An arrangement for introducing and implanting the electrode(s) of an endocardial implantable cardiac lead at a cardiac implantation site within a heart chamber or vessel. An elongated guide body formed of flexible material and extending between a guide body proximal end and a guide body distal end is advanced transvenously to position the guide body distal end in relation to the cardiac implantation site. Guide body tracking means is coupled with the lead distal end for receiving and slidingly engaging the guide body to allow the cardiac lead to be advanced along the guide body until the electrode is positioned at the cardiac implantation site. Pusher means formed of an elongated pusher body of flexible material extends between a pusher body proximal end and a pusher body distal end and has a cardiac lead engaging means for engaging the cardiac lead at or adjacent the distal lead end. The pusher body has sufficient column strength to be advanced alongside the guide body and lead body with the cardiac lead engaging means engaging and slidingly advancing the guide body tracking means and the cardiac lead distally along the guide body to thereby allow the cardiac lead to be advanced along the guide body until the electrode is positioned at the cardiac implantation site. The lead body is released from the lead engaging means and fixed, if the lead includes a fixation mechanism. The pusher means is retracted by retraction of the pusher body.
