    摘要: Visualization and treatment of tissue regions via percutaneous access methods and devices are described herein. The system may include a deployment catheter and an expandable imaging hood. In use, the hood may be introduced from outside the patient body through a percutaneous incision and advanced through a subxiphoid pathway, for example, to the heart where entry may be accomplished via an incision through the left atrial appendage. Once within the heart, the hood can be advanced to any chamber to visualize and/or treat tissue by placing the hood against the tissue to be imaged and pumping translucent fluids into the hood until the fluid displaces any blood, thereby leaving a clear region of tissue to be imaged via an imaging element in the deployment catheter. Additionally, any number of therapeutic tools can also be passed through the deployment catheter and into the imaging hood for treating the tissue of interest.

    摘要: A synthesis method is set forth for forming a physiologically acceptable composition comprising a plurality of physiologically acceptable hydrogel particles which have been swollen in a water solution containing a low molecular weight water soluble organic compound, the solution having the ability to swell the particles. The concentration of the organic compound is such that the resulting particles can be inserted, without use of a carrier liquid utilizing a hand driven hypodermic syringe. The swollen hydrogel particles are substantially insoluble in body fluids. The method comprises contacting not fully swollen hydrogel particles with the organic compound and with water.

    摘要: A device and method of use for securing and coring of tumors within the body during a biopsy of the tumor, specifically breast tumors. An adhesion probe for securing the tumor is described. The probe secures the tumor by piercing the tumor and providing a coolant to the distal tip to cool the tip. The cooled tip adheres to the tumor. A coring instrument adapted for cutting a core sample of the tumor is described. The instrument is provided with a cannula that can cut a core sample of the tumor. The instrument is adapted for use with the probe with the probe fitting within the cannula. The instrument can be used in conjunction with the probe to secure and core a sample of the tumor for biopsy.

    摘要: An apparatus for deploying a guidewire across a complex lesion in a vessel formed by a wall and normally having a lumen extending therethrough including an elongate flexible catheter having proximal and distal extremities and having a plurality of separate spaced apart lumens therein extending from the proximal extremity into a location at least adjacent the distal extremity. A handle is mounted on the proximal extremity of the flexible elongate catheter. A flexible elongate sheath is mounted on the catheter and has a proximal portion, a transition portion and a distal portion with the transition portion being in the vicinity of the distal extremity of the catheter. The sheath has a lumen therein extending from the proximal portion through the transition portion and through the distal portion. First and second devices are slidably mounted in the spaced-apart lumens of said catheter. The lumen in the distal portion of said sheath has a size so that it can accommodate only one at a time of the at least two devices.

    摘要: A self-supporting retractor for holding open an anatomic space for performing endoscopic surgical procedures. The retractor includes a substantially rigid elongate member, having proximal and distal ends, and having an arch shape or "C" cross-section, defining a passage and longitudinal working window along the retractor. The distal end is preferably rounded or streamlined to facilitate insertion along a dissected space with minimized tissue trauma, and may include a hooded portion enclosing the passage at the distal end. The elongate member may include radially-cooperating segments or self-retaining edges to maximize the area of the space held open by the retractor. The retractor may include a handle or a pivotable finger grip on the proximal end to manipulate the retractor during use, and/or a curved handle to tent the incision providing access to the dissected space. The retractor may also include a channel member for an endoscope, integrally formed or pivotally attached to the retractor.

    摘要: A composition for injecting into tissues surrounding the urethra or ureter. It comprises a plurality of physiologically acceptable solid polymer particles dispersed in a physiologically acceptable biodissipatable liquid carrier. The polymer comprises a hydrophilic component. The solid polymer particles are a glycerine insoluble poly(ethylene oxide) (PEO) polymer or copolymer or a mineral oil insoluble polyvinylpyrrolidone polymer or copolymer. The plurality of solid polymer particles are capable of hydrating and upon hydration of absorbing at least about 30% water and of swelling to a predetermined volume. The solid particles are substantially insoluble in body fluids and substantially insoluble in and non-swellable by the liquid carrier. The composition is especially suitable in treating patients with urinary incontinence and patients with vesicoureteral reflux via injection into the tissues around the urethra or ureter.
