摘要： Devices and methods for increasing blood perfusion within the myocardium of the heart by wounding the myocardium and applying growth factors to promote vascularization. One method includes driving spikes into the myocardium from within the heart, with the spikes being formed of biodegradable material, containing releasable growth factors, and having lumens and side holes. An alternative method utilizes non-biodegradable spikes with lumens and side holes. Another method promotes vascularization by injecting angiogenic material into holes bored into the myocardium, leaving a patent hole. Another method includes injecting angiogenic material into the myocardium without boring, leaving no patent hole. The angiogenic material can be carried in a biodegradable adhesive. Revascularization of the myocardium is promoted in one method by externally wounding the heart and applying a patch including growth factors.

This catheter may include a first elongate shaft having a proximal end and a distal end, a second elongate shaft having a proximal end and a distal end, a first sharpened distal tip disposed at the distal end of the first shaft and a second sharpened distal tip disposed at the distal end of the second shaft. The first shaft and the second shaft may also be secured to one another at the distal end of the first shaft according to the invention. Still further, according to other teachings, the first sharpened distal tip may extend beyond the second sharpened distal tip in a catheter that practices the invention.

摘要： An apparatus and method for exchanging a first intravascular catheter of the over-the-wire type, such as an angioplasty balloon dilation catheter, with a second intravascular catheter of the over-the-wire type while leaving a guide wire in position in a desired vascular site. The apparatus is used in conjunction with a conventional guide catheter, a guide wire, and an interventional catheter that can be positioned intravascularly through a lumen of the guide catheter over the guide wire and past a distal end of the guide catheter to a desired vascular site. The apparatus includes an elongate shaft that can be removably positioned in the lumen of the guide catheter and has an actuation capability therein. A trapping member is located at a distal portion of the elongate shaft for releasably fixing the relative position of the guide wire with respect to the guide catheter. The fixing and releasing of the trapping member is effected by the actuation capability from outside the body of the patient so that a first interventional catheter can be exchanged for a second interventional catheter by trapping the guide wire position relative to the guide catheter while withdrawing the first interventional device and replacing it with another.
