摘要: Spinal spacers 20 are provided for fusion of a motion segment. The spacers include a load bearing member 21 having a wall 22 sized for engagement within a space between adjacent vertebrae to maintain the space and an effective amount of an osteogenic composition to stimulate osteoinduction. The osteogenic composition includes a substantially pure osteogenic factor in a pharmaceutically acceptable carrier. In one embodiment the load bearing member includes a bone graft impregnated in an osteogenic composition. In another embodiment, the osteogenic composition 30 is packed within a chamber 25 defined in the graft. Any suitable configuration of a bone graft is contemplated, including bone dowels, D-shaped spacers and cortical rings.

摘要: Spinal spacers 20 are provided for fusion of a motion segment. The spacers include a load bearing member 21 having a wall 22 sized for engagement within a space between adjacent vertebrae to maintain the space and an effective amount of an osteogenic composition to stimulate osteoinduction. The osteogenic composition includes a substantially pure osteogenic factor in, a pharmaceutically acceptable carrier. In one embodiment the load bearing member includes a bone graft impregnated in an osteogenic composition. In another embodiment, the osteogenic composition 30 is packed within a chamber 25 defined in the graft. Any suitable configuration of a bone graft is contemplated, including bone dowels, D-shaped spacers and cortical rings. A spinal spacer 300 for engagement between vertebrae is also provided which includes a body 301 formed of a bone composition. The body 301 includes a first end 311, an opposite second 315 end, a superior face 335 defining a superior vertebral engaging surface 337 and an inferior face 338 defining an inferior vertebral engaging surface 340. At least one of the vertebral engaging surfaces defines a set of migration resistance grooves 350. Each of the grooves 350 includes a first face 355 defining an angle of no more than about 90 degrees relative to the engaging surface 340 and a second opposing sloped face 360. The first and second faces 355, 360 define an arcuate pocket 370 therebetween for trapping vertebral bone to resist migration of the spacer 300. In one embodiment, the grooves 350 are arranged in series in that all of the second faces 360 slope in the same direction.

摘要: A method and instrumentation particularly adapted for disc space preparation from an anterior approach to the spine. In one aspect, an expandable template is provided having guides to guide a cutting device for bilateral formation of openings in the disc space. In another aspect, an improved guide member is provided for guiding a cutting tool. Still further, the invention provides an improved double barrel guide sleeve with a central distraction extension and lateral non-distracting extensions. Optionally, the guide sleeve includes windows and covers to selectively cover the windows. An improved reamer with an internal chamber and optional modular coupling is also provided. A depth stop is provided to selectively engage a tool shaft and a guide sleeve to control tool penetration into the disc space. A method of using the disclosed instruments is also provided.

摘要: An interbody fusion device for engagement between vertebrae includes a pair of lateral spacers for spacing the vertebrae, and a connecting member adapted to couple together the lateral spacers when inserted between the vertebrae. The connecting member, individual lateral spacers, or the entire spinal spacer can be made of bone in order to promote fusion of the vertebrae. The modular construction permits use of bone segments otherwise unsuitable due to size or strength in stable fusion devices.

摘要: An interbody fusion device in one embodiment includes a tapered body defining a hollow interior or chamber for receiving bone graft or bone substitute material. The body defines exterior threads which are interrupted over portions of the outer surface of the device. The fusion device includes truncated side walls so that on end view the body takes on a cylindrical form. In another embodiment, the tapered body is solid and formed of a porous biocompatible material having sufficient structural integrity to maintain the intradiscal space and normal curvature. The material is preferably a porous tantalum composite having fully interconnected pores to facilitate complete bone tissue ingrowth into the implant. In further embodiments, the fusion devices are provided with osteogenic material to facilitate bone ingrowth. A cap is also provided to block the opening of hollow fusion devices. The cap includes an occlusion body and an elongated anchor. In some embodiments the anchor includes a lip which is engageable to openings in the body wall. Tools are also provided for manipulating caps for interbody fusion devices. In one embodiment the tool includes a pair of prongs each having facing engagement surfaces for engaging the fusion device, and a shaft slidably disposed between the prongs. The shaft has a first end defining a cap-engaging tip for engaging a tool hole in the cap. In one embodiment the cap engaging tip defines threads. In another embodiment the prongs include a pair of releasing members on each of the facing engagement surfaces. The releasing members have a height and a width for being insertable into apertures in a body wall in the fusion device to disengage the elongate anchors from the apertures.

摘要: The present disclosure provides improved prosthetic joints and a nucleus supplement device for a spinal disc without violating its annulus fibrosus. In one example, a prosthetic joint includes: a tip adapted for mating against a superior articular process of an inferior vertebra, and the tip acts as at least a spacer between the superior articular process and an inferior articular process of a superior vertebra; and an body for mating against a hole in the superior vertebra, and the body comprises a surface that is uneven along its entire length. In another example, a method for restoring motions of a joint includes: providing a body for a prosthetic joint; threading a surface of the body, and the threading extends an entire length of the body; and creating an internal cavity in the prosthetic joint.

摘要: An articular disc prosthesis and method of implanting the same within an intervertebral space between adjacent vertebral bodies. The prosthesis includes a pair of articular components and an articular ball disposed therebetween. Each of the articular components includes an outer shell portion and a removable inner insert portion. The insert portion includes a concave articular surface sized and shaped to receive a portion of the articular ball to provide articulating motion between the articular components. The outer shell portion includes a central hemi-cylindrical portion, a pair of laterally extending flanges, and an axially extending lip. Following removal of the natural intervertebral disc, a pair of hemi-cylindrical recesses are formed along a central region of the adjacent vertebral bodies to a predetermined depth. The prosthesis is implanted within the prepared disc space by axially displacing the hemi-cylindrical central portions of the articular components along the hemi-cylindrical recesses in the vertebral bodies. The lateral flanges and the axial lip of the articular components bear against the endplates of the adjacent vertebral bodies to stabilize the prosthesis and to prevent subsidence.

At least one of the convex and concave articular surfaces includes at least one surface depression that is configured to facilitate removal of matter disposed between abutting portions of the articular surfaces. In one embodiment of the prosthetic joint, each of the articular components has a vertebral bearing surface and a flange extending therefrom that is configured to penetrate a corresponding one of the first and second vertebrae, with the flange defining at least one opening extending therethrough to permit bone through-growth.
