Amide derivatives represented by the following general formula (1) which have a C5 alpha receptor antagonism, wherein each symbol has the meaning as defined in the description. These amide derivatives, optically active isomers thereof or pharmaceutically acceptable salts of the same are useful as preventives and remedies for diseases or syndromes caused by inflammation induced by C5 alpha [for example, immunological diseases such as rheumatism and systemic lupus erythematosus, allergic diseases such as sepsis, adult respiratory distress syndrome, chronic obstructive pulmonary disease and asthma, atherosclerosis, heart infarction, brain infarction, psoriasis, Alzheimer's disease and important organistic breakdown (for example, pneumonia, nephritis, hepatitis, pancreatitis) induced by leukocyte activation caused by ischemic reperfusion, burn or surgical invasion].
