    摘要: This document discusses, among other things, an apparatus comprising an implantable cardiac depolarization sensing circuit, an electrical stimulation circuit, and a pacing mode controller. The pacing mode controller is configured to deliver pacing therapy according to a first pacing mode that is a normal operating mode, and to deliver pacing therapy according to second and third pacing modes. The second and third pacing modes increase mechanical stress on at least a particular portion of the ventricle as compared to the pacing therapy delivered during the first pacing mode. The pacing mode controller alternates between the second and third pacing modes when pacing is changed from the normal operating mode to a stress augmentation mode. The pacing mode controller suspends the change from the normal operating mode to the stress augmentation mode when a condition to prevent the change is detected.

    摘要: Devices and methods for detecting disordered breathing involve determining that the patient is asleep and sensing one or more signals associated with disordered breathing indicative of sleep-disordered breathing while the patient is asleep. Sleep-disordered breathing is detected using the sensed signals associated with disordered breathing. The sensed signals associated with disordered breathing may also be used to acquire a respiration pattern of one or more respiration cycles. Characteristics of the respiration pattern are determined. The respiration pattern is classified as a disordered breathing episode based on the characteristics of the respiration pattern. One or more processes involved in the detection of disordered breathing are performed using an implantable device.

    摘要: A medical device lead includes an insulative lead body, outer and inner conductive coils, and a flexible core assembly. The outer conductive coil extends through the lead body and is coupled to a first electrode at a distal end of the outer conductive coil. The inner conductive coil extends coaxially with the outer conductive coil, is coupled to a second electrode at a distal end of the inner conductive coil, and includes a central lumen. The flexible core assembly is disposed in the central lumen and is comprised of a material that has a saturation magnetization of at least about 1.5 T and a relative permeability of greater than one. The flexible core assembly includes a positioning interface configured for manipulation of the flexible core assembly such that the flexible core assembly translates through the central lumen during insertion and extraction of the flexible core assembly.

    摘要: A gas therapy system involves sensing the blood gas concentration of the patient and adapting a gas therapy based on the sensed gas concentration. Disordered breathing may be detected bases on blood gas concentration, and gas or cardiac electrical therapy may be adapted to treat the detected disordered breathing. One or more of sensing the blood gas concentration, detecting disordered breathing, or adapting the therapy may be performed at least in part implantably. The gas therapy is delivered to the patient through an external respiratory device, such as a positive airway pressure device.
