    Abstract: Systems, devices, and methods for endoscopic access to a target region in a patient pancreaticobiliary system from an entry site of a stricture is disclosed. The stricture can be opened up using an radio-frequency (RF)-based approach by delivering RF energy to a working head of a steerable elongate instrument positioned at the entry site, or a mechanical puncture-based approach by applying force to a working head made of material with substantial stiffness. A machine learning (ML) model can be trained to determine a proper pancreaticobiliary access method for the patient, such as between the RF-based approach and the mechanical puncture-based approach. At least a distal portion of the steerable elongate instrument can be advanced into the pancreaticobiliary region to perform a diagnostic or therapeutic operation therein.

    Abstract: A surgical method including inserting a puncture wire and sheath in a first direction through a working channel in an ureteroscope and advancing the puncture wire from the sheath into a selected papilla and through a flank of a patient so the puncture wire has an emergent segment extending beyond the flank. A catheter is advanced over the emergent segment of the puncture wire outside the flank in a second direction opposite a direction of renal and flank puncture and into the flank in the second direction, the catheter having a lumen and a radiopaque portion to provide visibility of a tip of the catheter via fluoroscopic imaging. The puncture wire is then removed from the catheter after the tip of the catheter is positioned in a desired region.

    Abstract: An endoscope in which a reusable portion is inserted nearly all the way into a hollow, pistol-grip handle of a single-use portion and the protruding part of the reusable portion and an adjacent part of the handle are covered with a flexible and resilient cap. The same cap covers the open end of the handle when the single-use portion is shipped from a manufacturing site. At a user's facility, a health professional tears the pouch, removes the cap from the handle, inserts the reusable portion, and covers the distal end of the reusable portion and an adjacent part of the handle with the same cap. Typically, a remote display connects to the endoscope wirelessly or through a cable, with a connector that fits through a hole in the cap configured to keep fluids from entering around the connector. The wireless transmission preferably uses a proprietary protocol to preserve medical data confidentiality.

    Abstract: A retractable syringe device includes: a handle including a hollow body for receiving a syringe through an opening of the handle, the hollow body including a syringe actuator that moves the syringe between a distal position and a proximal position; and an instrument distally extending from the handle, the instrument including a channel extending through the instrument, the channel configured to receive a needle coupled to the syringe. When the syringe is in the distal position, a distal end of the needle distally extends from the instrument, and when the syringe is in the proximal position, the distal end of the needle is retracted into the channel. The handle of the device can include a fixed or removable structure for removably coupling to a proximal end of a shaft of an endoscope.
