    摘要: A device for introducing a catheter into a vessel through a puncture in a vessel and for sealing the puncture. The device includes an elongated body having a proximal end and a distal end sized to be positioned within a tissue site which includes the puncture. The elongated body includes a utility lumen sized to allow a catheter to be delivered through the utility lumen. The utility lumen is positioned within the elongated body so positioning the elongated body within the tissue site allows a catheter delivered through the utility lumen to enter the vessel. The elongated body also includes a closure lumen having an entrance port. A closure composition can be delivered through the entrance port into the closure lumen. The closure lumen also includes an exit port adjacent the distal end of the elongated body. The closure composition delivered into the closure lumen can be delivered through the exit port to the tissue site adjacent the puncture.

    摘要: The present invention provides apparatuses and methods for performing minimally invasive diagnostic and surgical procedures inside of a beating heart. An assembly for use in performing such a medical procedure includes a port and a fluid transport device. The port has a housing configured for insertion through a chamber wall of a heart chamber, at least one valve disposed in the housing, and an inlet connected to the housing. The inlet has an inlet passage in fluid communication with a lumen of the housing. The fluid transport device has an end that attaches to the inlet of the port, another end that attaches to a fluid source, and a fluid channel therebetween to pass a fluid from the fluid source to the heart chamber. A method of preparing a beating heart for a diagnostic or medical procedure comprises the steps of inserting a cardiac port through a chamber wall of the beating heart and pressurizing the heart chamber with blood from the patient's artery. Repair of the heart's mitral valve can be effected by a stapled annuloplasty procedure. Where sutures are used in the diagnostic or surgical procedure, a suture cutting device for cutting a suture inside a beating heart can be used. The suture cutting device can include an elongated body having a lumen, a wall member extending into the lumen, and a knot-receiving chamber defined by a first area on a first side of the wall member. The wall member defines a channel therethrough dimensioned to permit passage of a suture and to prevent passage of a knot. A cutting implement is movably disposed in the lumen in a second area on a second side of the wall member opposite the first side. The cutting implement moves between a first position and second position to cut the suture. Movement of the cutting implement is limited to ensure that the cutting implement does not cut through the suture knot.

    摘要: An endoscope for insertion into a patient's inner body cavity has a shell with lumens for (1) illuminating a lens and (2) passing light from an imaging (or objective) lens, each lens being disposed on the distal face of the shell. The shell also has a lumen, preferably off-round, for receiving a resilient disposable core. The resilient core has passageways for introducing a pressurized fluid (e.g. air or water) to selective ones of the lenses to clean the lenses. The core may also have another passageway for removing material from the patient's inner body cavity for analysis. This passageway may communicate with two (2) conduits for removing specimens from the body cavity (1) as by a vacuum or (2) as by an instrument. The second conduit may be closed except when the instrument is to be inserted into a patient's inner body cavity. The core may be drawn through the shell lumen to position the core in the lumen. A cleaning member may be attached to the core to clean the shell lumen as the core is drawn through the lumen. Alternatively, a leader applied to the core may be drawn with the core through the shell lumen and then removed from the core. Sealing members attached to the core may also be drawn through the lumen with the core to seal the core. Alternatively, members may be sealed to the core or shell after the sealing operation. A cover on a core end may alternatively be removed after the core has been drawn through the shell lumen.
