Abstract: An extra-cardiovascular implantable cardioverter defibrillator (ICD) having a low voltage therapy module and a high voltage therapy module is configured to select, by a control module of the ICD, a pacing output configuration from at least a low-voltage pacing output configuration of the low voltage therapy module and a high-voltage pacing output configuration of the high voltage therapy module. The high voltage therapy module includes a high voltage capacitor having a first capacitance and the low voltage therapy module includes a plurality of low voltage capacitors each having up to a second capacitance that is less than the first capacitance. The ICD control module controls a respective one of the low voltage therapy module or the high voltage therapy module to deliver extra-cardiovascular pacing pulses in the selected pacing output configuration via extra-cardiovascular electrodes coupled to the ICD.

Abstract: Techniques are disclosed for measuring an analyte in a biological system. A system may include a medical device with an electrochemical sensor configured to sense the concentration of a plurality of analytes present in a biological system. Processing circuitry of the system may retrieve, identify, and process a respective signal from a respective work electrode to determine the concentration of a respective analyte. The system may further include an implantable medical device configured to sense a cardiac electrogram (EGM). In some examples, the system may be configured to determine one or more patient-specific relationships between the respective signals of the electrochemical sensor and the cardiac EGM during a first period of time. Based on the patient-specific relationships, the system may estimate concentrations of the one or more analytes corresponding to the respective signals based on the cardiac EGM of the patient collected over a second period of time.

Abstract: A method of creating a shunt between a right atrium and a left atrium of a mammalian heart including puncturing an atrial septum between the right atrium and the left atrium to create a shunt. An ablation device having balloon is advanced at least partially through the shunt. The balloon is inflated and configured to thermally isolate the atrial septum from blood within the left atrium and the right atrium. Ablation energy is delivered to ablate the atrial septum.

Abstract: The disclosure describes systems and techniques for detection of pump thrombosis in mechanical circulatory support (MCS) devices. An example pump thrombosis detection system includes a transducer and processing circuitry. The transducer may be configured to generate a signal representative of a mechanical wave from a mechanical circulatory support device. The processing circuitry is communicatively coupled to the transducer. The processing circuitry may be configured to determine an indication of pump thrombosis based on the signal and, based on the indication of pump thrombosis, control the pump thrombosis detection system to at least one of generate an alert or initiate an intervention.

Abstract: The exemplary systems and methods may monitor one or more signals to be used to assess the hemodynamic status of a patient. The one or more signals may be used to calculate, or determine, a plurality of pulse transit times. The plurality of pulse transit times may be used to determine hemodynamic status values that may be indicative of a patient's aggregate hemodynamic status.
