    摘要: A surgical instrument for cutting and applying a plurality of surgical fasteners in the same direction to body tissue includes a frame having a proximal end and a distal end, with a handle positioned at the proximal end and an end effector positioned at the distal end. The end effector is shaped and dimensioned for supporting a cartridge housing and an anvil. The cartridge housing and anvil are relatively movable between a first spaced apart position and a second position in close approximation with one another, and the cartridge housing includes a knife structure adapted for cutting tissue by moving between the cartridge housing and the anvil. The instrument also includes a firing mechanism associated with the end effector and the cartridge housing for selective actuation of the knife structure and surgical fasteners for movement in the same direction. The knife structure includes a rearwardly facing coupling member shaped and dimensioned for engaging a distal end of the firing mechanism as the cartridge housing is loaded into the end effector of the surgical instrument.

    摘要: A surgical instrument is adapted for applying a plurality of surgical fasteners to body tissue. The surgical instrument includes a frame having a proximal end and a distal end, with a handle positioned at the proximal end and an end effector positioned at the distal end. The end effector is shaped and dimensioned for supporting a cartridge housing and an anvil, the cartridge housing and anvil being relatively movable by actuation of a closure mechanism. The surgical instrument further includes a firing mechanism associated with the end effector and the cartridge housing for selective actuation of the surgical fasteners features. The surgical instrument also includes a retaining pin for enclosing tissue between the anvil and the cartridge housing. The retaining pin is moved by actuation of the closure mechanism from an open first position to a closed second position. A manual actuator is associated with the retaining pin for moving from the second position back to the first position and alternatively moving the retaining pin from the first position to the second position.

    摘要: A surgical trocar has a cannula assembly including a sleeve having a proximal end, a distal end and a passageway therethrough. A housing having an opening is located at the proximal end of the sleeve and is aligned with the passageway of the sleeve. The trocar also includes an obturator assembly which is insertable in the cannula assembly. The obturator assembly includes a handle having an opening therein and a flexible contact portion which is outwardly deflectable from the handle. The obturator assembly also includes an obturator having a proximal end connected to the handle and a distal end opposite the proximal end. A penetrating tip is located at the shaft distal end for penetrating tissue. A protector having a distal end is slidably disposed around the obturator shaft for covering the penetrating tip. The protector has an aperture at its distal end for permitting the penetrating tip to pass therethrough. The protector proximal end is movable against the flexible contact portion of the obturator handle to expose the penetrating tip when a proximal force is asserted against the protector.

    摘要: A surgical instrument for penetrating bodily tissue including an imaging element providing the capability for simultaneous visualization during penetration is disclosed. A transparent portion of the imaging element extends from the distal end of an elongated, substantially hollow shaft. The imaging element is shaped to enlarge an opening as the instrument is advanced into bodily tissue, and an endoscope can be inserted through the shaft adjacent the imaging element to visualize the penetration of tissue as the instrument is advanced. The imaging element may be substantially hollow, and have an annular interior region inside the shaft distal end. The endoscope can then be positioned to rest on the annular region to prevent glare from the reflection of light traveling from the endoscope to the window surface. The imaging element may also have a blade for facilitating the advance of the instrument through tissue. The blade not only has a sharp, linear cutting edge, but also a blunt edge for preventing inadvertent cutting of tissue. The blunt edge is preferably located near the tip and base of the imaging element. Additionally, the imaging element is generally conical and the penetrating surface has a region extending at an obtuse angle relative to the exterior surface of the conical element, and a second region extending generally parallel to the exterior surface of the conical element.
