摘要:
An apparatus, probe and method for a cryogenic system are described. According to certain embodiments of the invention there is provided an apparatus for cryosurgery comprising: an exhaust line configured to receive cryogen from a probe; and a vacuum source configured to be in fluid communication with the exhaust line.

Cryotherapeutic systems configured for refrigerant recompression and associated devices, systems, and methods are disclosed herein. A cryotherapeutic system configured in accordance with a particular embodiment includes a high-pressure line, a low-pressure line, a recompression unit, and a cryo-catheter. The recompression unit increases the pressure of refrigerant from the low-pressure line to the high-pressure line to a treatment pressure sufficient for cryogenic alteration of tissue. The high-pressure line and the low-pressure line include, respectively, a first connector and a second connector. The cryo-catheter includes a shaft, a supply lumen in the shaft that is coupled to the first connector, an exhaust lumen in the shaft that is coupled to the second connector, and a cryo-applicator attached to the shaft having a cooling chamber configured to receive refrigerant from the supply lumen and to return refrigerant via the exhaust lumen to the low-pressure line.

Devices and methods for cooling vessel walls to inhibit restenosis in conjunction with medical procedures such as coronary artery angioplasty. Stenosed vessel walls can be cooled prior to angioplasty, after angioplasty, or both. The invention is believed to inhibit restenosis through cooling to a temperature near freezing, preferably without causing substantial vessel wall cell death.

Post-angioplasty hyperplasia in blood vessels is treated using a cryosurgical balloon catheter. The balloon catheter is positioned at a target region within the blood vessel, and the balloon inflated by expanding a cryogenic fluid, such as liquid nitrogen, across an expansion orifice into a balloon. The balloon will be constructed so that cooling is achieved primarily in the central regions of the balloon, with the proximal and distal regions being less cold and acting to insulate adjacent regions of the blood vessel from excessive cooling.

The invention features an apparatus and method for protecting tissue from freeze damage during cryosurgery. According to the method, a cryoprotective agent is introduced to a tissue to be protected in an amount sufficient to protect the tissue from damage during cryosurgery, and cryosurgery is performed on tissue to be destroyed.

A closed end surgical cryoprobe instrument with probe shaft diameter of 3 millimeters or less is made to achieve and maintain freezing zone temperatures close to that of the liquid cryogenic refrigerant. Using sub-cooled liquid nitrogen at approximately -208.degree. C. freezing zone temperatures as low as -206.degree. C. can be achieved in under 1 minute. The liquid nitrogen supply tube is provided with a plurality of small vent holes to vent gas formed or present in the refrigerant supply tube to the return refrigerant flow channel. The vent holes also allow small amount of liquid nitrogen to vent into the return flow channel to further reduce the temperature differential between the sub-cooled liquid nitrogen supply and the counter-current flowing return refrigerant.

Devices for therapeutic nasal neuromodulation and associated systems and methods are disclosed herein. A system for therapeutic neuromodulation in a nasal region configured in accordance with embodiments of the present technology can include, for example, a shaft and a therapeutic element at a distal portion of the shaft. The shaft can locate the distal portion intraluminally at a target site inferior to a patient's sphenopalatine foramen. The therapeutic element can include an energy delivery element configured to therapeutically modulate postganglionic parasympathetic nerves at microforamina of a palatine bone of the human patient for the treatment of rhinitis or other indications. In other embodiments, the therapeutic element can be configured to therapeutically modulate nerves that innervate the frontal, ethmoidal, sphenoidal, and maxillary sinuses for the treatment of chronic sinusitis.

A transseptal access stability system (TASS) and method of use thereof is disclosed. The TASS includes a venous sheath for reducing the likelihood of complications that may arise when performing transseptal punctures. The TASS uses either suction force or cryo-based energy for securing the fossa ovale against the rim of the orifice of the venous sheath. A method of performing a transseptal puncture using the presently disclosed TASS also is disclosed.

Devices and methods for cooling vessel walls to inhibit restenosis in conjunction with medical procedures such as coronary artery angioplasty. One catheter device includes a distal tube region having coolant delivery holes radially and longitudinally distributed along the distal region. In some devices, holes spray coolant directly onto the vessel walls. In other embodiments, a balloon or envelope is interposed between the coolant and the vessel walls and the coolant returned out of the catheter through a coolant return lumen. Some direct spray devices include an occlusion device to restrict blood flow past the region being cooled. Pressure, temperature, and ultrasonic probes are included in some cooling catheters. Some cooling catheters are coiled perfusion catheters supporting longer cooling periods by allowing perfusing blood flow simultaneously with vessel wall cooling.
