A prosthetic hip joint includes a ceramic hip head having a bore which defines a surface, wherein the bore surface has a layer of malleable material disposed on at least a portion thereof. The surface of the bore includes surface imperfections in the form of peaks and valleys. The prosthetic hip joint can also include a femoral component having a trunnion with a tapered surface adapted for friction fit insertion into the bore of the hip head. The layer of material, such as pure titanium, distributes a load on the joint reducing localized stress points produced by opposing asperity peaks in the load-bearing surfaces of the bore and trunnion.

Pieces of metal are joined with silver material in a two-stage heating process. In one application, a brazed porous coating of metal particles on a metal substrate of a device is formed by interposing a silver material between the substrate and the particles, and placing the particles onto the substrate. The device is heated to a first temperature slightly below the melting point of the silver material, then briefly heated to a second temperature above 1100.degree. C. to rapidly melt the silver material to wet the particles and substrate and to generate alloying among the silver material, the particles, and the substrate. The device is allowed to cool to solidify the alloy and unite the particles and substrate to form a porous coating on the device. Also disclosed are a prosthetic device having a metal shaft for insertion into a bone canal, and a porous coating of metal particles brazed to the metal shaft by a silver material. A porous coating preparation to be applied to a metal substrate, including metal particles, silver material, and a vaporizable binder for initially securing the particles to the substrate is further disclosed.

Surgical implants, especially endoprosthetic orthopaedic implants and sutures, are rendered antimicrobial by the presence of a bioerodible metallic silver component, especially a surface coating, which provides in vivo a sustained release of silver ions in a concentration sufficient to provide a localized antimicrobial effect but insufficient to cause significant damage to connective tissue. Latently bioerodible silver components of an implant can be activated by, for example, abrasion, heating to above about 180.degree. C. or, especially, contact with hydrogen peroxide.

A metal implant for use in a surgical procedure is provided with a surface layer that is integral with the metal substrate, and which incorporates a biocidal material. The surface layer may be grown from the metal substrate, by anodising, and the biocidal material incorporated in it by ion exchange. Alternatively the layer may be deposited by electroplating, followed by diffusion bonding so as to become integral with the metal substrate. In either case, silver is a suitable biocidal material; and both the release rate and the quantity of biocidal material should be low to avoid toxic effects on body cells. Electropolishing the surface before formation of the surface layer is also beneficial, and this may be achieved by electropolishing.

A flanged interbody spinal fusion implant for insertion at least in part between adjacent vertebral bodies has opposed upper and lower surfaces adapted to contact each of the adjacent vertebral bodies from within the disc space, a leading end for insertion between the adjacent vertebral bodies, and a trailing end opposite the leading end. The trailing end has a height greater than the maximum height of the disc space forming a flanged portion adapted to overlie a part of the anterior aspects of the vertebral bodies adjacent and proximate the disc space to be fused. The flanged portion has at least two bone screw receiving holes, at least one each adapted to overlie each of the adjacent vertebral bodies at least in part. The implant has at least one locking element to lock one or more of the bone screws inserted in the bone screw receiving holes. Instrumentation for inserting the implant, forming screw holes in the vertebral body, and for locking the locking elements are disclosed.
