摘要:
Methods and devices are provided to allow for easy manipulation of a configuration and/or an orientation of seal elements of a surgical access device by an end user. In one exemplary embodiment a surgical access device is provided that includes a retractor configured to be positioned within a surgical incision to provide access to a body cavity, seal elements disposed within a lumen of the retractor, and adjustment mechanisms in communication with the seal elements and which are configured to manipulate an orientation of one or more of the seal elements. Types of adjustment mechanisms capable of manipulating the seal elements as desired include inflatable bladders and electroactive polymers. Exemplary methods for providing easy manipulation of the configuration and/or orientation of seal elements of a surgical access device on location are also provided.

Methods and devices are provided to allow for easy customization of a surgical access device by an end user. In one exemplary embodiment a surgical access device is provided that includes a plurality of elongate seal elements that are configured to form a sealed configuration between a surgical site and an outside environment. The seal elements can be disposed in a surgical opening and can mate directly to each other with the outermost seal elements mating directly to tissue of the opening. The seal elements can include one or more mating elements to provide the desired mating. The seal elements can also include sealable openings that are configured to receive surgical instruments for use at the surgical site. The sealable openings maintain the desired seal throughout the course of a surgical procedure. Exemplary methods for providing custom configurations on location are also provided.

A self contained motor-powered disposable loading unit for use with a surgical cutting and stapling apparatus. The disposable loading unit may contain a battery that is retained in a disconnected position when the disposable loading unit is not in use and is moved to a connected position when the disposable loading unit is coupled to the surgical cutting and stapling apparatus to permit the motor to be selectively powered thereby. Indicators may be supported on the disposable loading unit to indicate when the axial drive assembly thereof is in a starting position and an ending position. Another indicator may be provided to indicate when the anvil assembly is in a closed position.

A piece of buttress material and an end-effector of a surgical instrument including features which can aid a surgeon in properly and quickly attaching the piece of buttress material to the end-effector. In various embodiments, a piece of buttress material can include retention features which can be engaged with portions of an end-effector to releasably retain the piece of buttress material to at least a portion of the end-effector. Similarly, an end-effector can include features configured to engage portions of a piece of buttress material to releasably retain the piece of buttress material to the end-effector. In at least one embodiment, more than one piece of buttress material can be releasably retained to an end-effector.

A surgical instrument for being endoscopically or laparoscopically inserted into a surgical site for simultaneous stapling and severing of tissue includes force adjusted spacing between an upper jaw (anvil) and a lower jaw (staple cartridge engaged to an elongate staple channel) so that the height of staple formation corresponds to the thickness of the tissue, yet does not exceed the height range that may be accommodated by the length of the staples. In particular, resilient structures may be formed into one or more of the elongate channel that supports the staple cartridge, the anvil that is pivotally attached to the elongate channel, and/or a firing member that includes a cutting surface (knife) that severs tissue between a top pin that engages the anvil and a lower foot that engage the elongate channel. The resilience responds to the force exerted by clamped tissue to vary the spacing between the anvil and the staple cartridge supported within the elongate channel.

A surgical instrument for use in connection with a pneumatically powered tool having a firing mechanism operably supported therein that is movable between an unactuated position and an actuated position. A pneumatically powered drive system operably communicates with the pneumatically powered tool and is configured to selectively apply at least a firing force to the firing mechanism of the pneumatically powered tool of sufficient magnitude to cause the firing mechanism to move from the unactuated position to the actuated position in response to a flow of gas from a source of gas that is fluidically coupled to the pneumatically powered drive system. At least one feedback device communicates with the pneumatically powered drive system for providing feedback concerning at least one of:(a) the magnitude of the firing force as the pneumatically powered drive system moves the firing mechanism from the unactuated to actuated position; and(b) a relative position of the firing mechanism as the firing mechanism is moved between the unactuated and actuated positions.

The present invention includes a surgical stapler having a staple cartridge, an anvil, and a cutting member having a cutting surface, wherein the cutting member is relatively movable with respect to the anvil and the staple cartridge. In at least one embodiment, the anvil is movable between an open position and a partially-closed position. When the anvil is in its partially-closed position, the anvil and the staple cartridge can be configured to capture tissue therebetween without fully compressing the tissue. In at least one embodiment, the anvil includes a proximal portion, a distal portion, and a middle portion intermediate the proximal portion and the distal portion. In these embodiments, the tissue can be captured between the proximal and distal portions of the anvil before the middle portion of the anvil is brought into contact with the tissue.

A retainer for assembling tissue thickness compensators to a surgical stapler can comprise a grip, a first surface for supporting a first tissue thickness compensator, a second surface for supporting a second tissue thickness compensator, and clips for aligning and attaching the retainer to the surgical stapler. The clips may align and attach the retainer to a staple cartridge of the surgical instrument. The clips may align the retainer with an anvil of the surgical instrument. An insertion tool may be used in combination with the retainer to insert the retainer into the surgical stapler and to push the tissue thickness compensators against the anvil and/or the staple cartridge of the surgical instrument.

An apparatus comprises a base and at least one indicator in communication with the base. The base comprises a housing and at least one slot. The at least one slot is shaped to receive a reusable component from a surgical instrument. The at least one indicator is in communication with the at least one slot. The base is configured to detect at least one characteristic related to the reusable component when the reusable component is placed into the at least one slot. Wherein the at least one indicator is configured to provide a signal to the user regarding the at least one characteristic.

Circular stapling instruments and anvil assemblies. The anvil assemblies may have collapsible anvil support members that may be inserted through an opening in a patient and then expanded to be attached to an anvil plate assembly that has a staple-forming surface thereon. The anvil support member is attachable to the anvil plate assembly in such a way that when the anvil assembly is coupled to the stapling head of a circular stapler, the staple-forming surface is in substantial registry with the staples supported in the stapling head. A variety of different anvil support members and anvil plate assemblies are disclosed.
