A method and apparatus for minimally invasive total joint replacement. The method involves sculpting the articular surface of a second bone that normally articulates with a first bone by attaching a bone sculpting tool directly or indirectly to the first bone with the tool in bone sculpting engagement with the articular surface of the second bone, and then sculpting the articular surface of the second bone with the joint reduced and moving one bone with respect to the other. An implant is placed to replace the articular surface of the second bone using an impaction device directly or indirectly attached to the first bone, wherein the force to place the implant is reacted by the second bone and the first bone.

Prosthetic joint replacement ensemble includes an artificial joint implant component having a non-bone-interfacing rough surface for ingrowth of fibrous tissue, preferably about a peripheral boundary. The component can be made of metal, and have the rough surface provided by porous metal coating. When the prosthesis is implanted, the extramedullary rough surface engenders ingrowth of soft fibrous tissue so that it forms, as it were, a reconstituted capsular “bag” in which wear debris from the joint, for example, ultra high molecular weight polyethylene wear debris, is trapped, thus keeping those particles from gaining access to supporting bone at the bone-prosthetic interface. And so, with mitigation of wear debris dispersion, debris wear-induced osteolysis can be ameliorated.

The present invention provides an improved prosthetic stem for implantation in a bone. Specifically, the present invention provides a prosthetic femoral stem having a strengthening rib protruding from the substrate thereof and being flush with a porous coating formed thereon. The strengthening rib is advantageously positioned so as to increase the cross-sectional moment of inertia of the prosthetic femoral stem and thereby decrease the stress on the anterio-lateral surface of the femoral stem. A protrusion in accordance with the present invention may also be utilized with a prosthetic femoral stem absent a porous coating. In such an embodiment, the height of the protrusion is such that the protrusion will be surrounded by the cement mantle formed when the prosthetic femoral component is cemented in a femoral canal.

An osteal prosthetic component of a prosthetic joint and a method of implanting an osteal prosthetic component in the medullary canal of a bone are disclosed. The osteal prosthetic component includes a stem for disposition within a medullary canal of a bone and a fluted wedge portion extending from the stem. The fluted wedge has wedge portions which define at least one flute between the wedge portions. An interference fit is formed between the wedge portions and the bone during implantation of the stem into the medullary canal. Movement of the osteal prosthetic component relative to the bone is thereby sufficiently limited to allow tissue growth into a tissue-ingrowth portion at the surface of the stem to substantially prevent micromotion of the stem relative to the bone.

A device for embedding in bone, in particular as a femoral hip-joint prosthesis, comprises a head part, a stem to be anchored in the bone, and a support collar terminating the stem at the head end. In order to make it easy for the surgeon, on reoperation, to remove the prosthesis stem from the bone tissue, the support collar can be removed. In addition, it can be combined with an anchor for attachment to the greater trochanter. By this means, on the one hand, the support collar is fixed in the envisaged position and, on the other hand, the greater trochanter participates with the adjacent lateral areas of the bone in taking up the load.

As a result, the conditions for the transmission of forces to the bone tissue in the proximal end section of the prosthesis stem are improved, it being possible at the same time to exert a favorable influence on the elasticity of the stem.
