    摘要: A vascular filter guide wire is disclosed for directing precision placement of a catheter proximate a blood vessel lesion and filtering particulate matter dislodged by treatment of the vessel. The guide wire includes an actuating mechanism, an elongated flexible core wire having a proximal end mounted to the actuating mechanism and a distal end for insertion through a vasculature to a position downstream of the restriction. A tubular flexible shaft is slidably disposed telescopically along the core wire and includes a proximal portion affixed to the actuating mechanism in movable relation to the core wire. The guide wire includes a collapsible filter at its proximal end to the distal portion of the shaft and, at its distal end, to the core wire. The filter deploys radially in response to axial movement of the core wire relative to the shaft so that it can trap particulate matter arising from treatment of the lesion.

    摘要: Suture clips and suture clip loading, delivery, locking and severing devices in both catheter and endoscopic embodiments are disclosed that permit the delivery and application of suture clips directly in contact with or in close approximation to sutured tissue. The catheter or endoscopic versions incorporate a collet cage with a plurality of flexible collet fingers that captivate a suture clip ring and a suture clip plug for assembly with a suture. An outer sliding sleeve constrains the collet fingers when distally advanced to minimize potential trauma during delivery and assembly of the suture clip in close proximity to the stitched tissue. Proximal retraction of the sliding sleeve coupled with the distal advancement of a pusher against cam surfaces on the inner walls of the collet fingers causes the collet fingers to open. The catheter is designed to hold and assemble the suture clip in a first position and release the suture clip and sever the suture ends proximal to the suture clip in a second position. Single pusher two-step embodiments and double pusher three-step embodiments are disclosed. Suture clip designs with plugs having heads for use with the suture clip delivery catheter are also disclosed. A suture clip loading device for loading suture clip components into the delivery system as well as a threader for threading sutures into the suture clip systems are also disclosed. A method of loading, delivering and deploying suture clips is further disclosed.

    摘要: The present invention provides a system (178) for delivering a therapeutic agent in combination with an implantable device (2) to maximize a therapeutic benefit offered by each. Preferably, the therapeutic agent is contained within a solid matrix form such as a pellet or gel to facilitate its handling and to regulate its rate of dissipation into the tissue after delivery. The implant device (2) is specially configured to receive and retain the matrix but permit blood to interact with the matrix so that the agent can be released to the blood in and around the device and the surrounding tissue. A delivery system (178) comprises an implant delivery device having an obturator (180) capable of piercing the tissue and an agent matrix delivery device (210) to place a matrix form, such as a pellet, into the interior of the implant (2) after it has been implanted. Preferably, the implant delivery device (180) and the matrix delivery device (210) are contained in one apparatus to facilitate delivery of the pellet into the embedded implant. The present invention is useful for treating tissue in any area of the body, especially ischemic tissue experiencing reduced blood flow. The present devices and methods are especially useful for treatment of ischemia of the myocardium. In treatment of the myocardium, the present implant device and matrix combination may be delivered surgically through the epicardium of the heart.

    摘要: Provided is an endoscopic suturing system for joining internal body tissues in a variety of procedures. The system comprises a suturing capsule releasably mountable to the distal end of an endoscope and capable of forming multiple stitches in tissue at a plurality of locations without requiring withdrawal of the capsule from the patient between stitches. Also enclosed is a suture lock to secure the placed stitch that is delivered by a device introduced through the working channel of the indwelling endoscope. Suitable control handles for the suturing capsule and for the suture lock delivery device positioned at the proximal end of the endoscope are provided to facilitate operation by the user.

    摘要: Implants and associated delivery systems for promoting angiogenesis in ischemic tissue are provided. The implants may be delivered percutaneously, thoracically or surgically and are particularly well suited for implantation into the myocardium of the heart. The implants are configured to be flexible so that they compress and expand with corresponding movement of the surrounding tissue into which they are implanted. The flow of blood into the implant and pooling of the blood in and around the implant leads to thrombosis and fibrin growth, a healing process that leads to angiogenesis in the tissue surrounding the implant. Additionally, the implants may contain an angiogenic substance or a thrombus of blood, preloaded or injected after implantation to aid in initiating angiogenesis.
