摘要: Described herein is a positioning tool having an elongate portion for carrying a medical implant. Also described is a method of positioning a medical implant using an elongate positioning tool. One form of the method includes positioning a medical implant on a distal portion of an elongate positioning tool, inserting the positioning tool with the implant thereon into a body cavity manipulating the positioning tool to position the implant into contact with tissue at an attachment location, attaching the implant to surrounding tissue at the attachment location, separating the implant from the positioning tool, and withdrawing the positioning tool from the body. In a preferred embodiment, the position of the implant is visually confirmed using an endoscope before the implant is attached to surrounding tissue. In one embodiment, the implant is a satiation device and the body cavity is the esophagus and/or stomach. In an alternative embodiment, an expansion structure on the distal end of the elongate portion expands and/or contracts the medical implant to facilitate positioning.

摘要: Various methods and devices are described for retaining a medical implant within a body cavity. According to one aspect, at least a portion of a medical implant is positioned within a body cavity, and a wall of the body cavity is re-shaped such that the re-shaped wall prevents migration of the medical implant out of the body cavity. The re-shaped body wall may form a tissue pocket, tunnel, or other barrier against migration of the implant.

摘要: Described herein are endoscopic plicators passed transorally into the stomach and used to plicate stomach tissue by engaging tissue from inside of the stomach and drawing it inwardly. In the disclosed embodiments, the tissue is drawn inwardly into a vacuum chamber, causing sections of serosal tissue on the exterior of the stomach to be positioned facing one another. The disclosed plicators allow the opposed sections of tissue to be moved into contact with one another, and preferably deliver sutures, staples or other means for maintaining contact between the tissue sections at least until serosal bonds form between them. Each of these steps may be performed wholly from the inside of the stomach and thus can eliminate the need for any surgical or laparoscopic intervention. After one or more plications is formed, medical devices may be coupled to the plication(s) for retention within the stomach.

摘要: A cell necrosis apparatus includes an introducer with a distal end sufficiently sharp to penetrate tissue. An energy delivery device has a first set of RF electrodes, a second set of RF electrodes and a third RF electrode. Each of electrodes has a tissue piercing distal end and is positionable in the introducer as the introducer is advanced through tissue. The first and second sets of RF electrodes are deployable with curvature from the introducer. The third RF electrode is deployable from the introducer with less curvature than the first and second sets of RF electrodes. The second set of RF electrodes is deployable a greater distance from the introducer than the first set of RF electrodes.

摘要: A method and apparatus for controlling the extent of tissue ablation are disclosed. The method utilizes delivering energy from an energy deliver device to a plurality of deployed electrodes and regulating energy delivered to the electrodes based on the temperature sensed by at least one remote elongate sensor element. A control device may be used to supply energy to the electrodes, to produce tissue ablation that advances from individual-electrode ablation regions to fill a combined-electrode ablation volume. Information from the sensor element(s) is used for determining the extent of ablation in the regions of the sensor elements. The supply of energy to the electrodes can thus be regulated to control the level and extent of tissue ablation throughout the combined-electrode volume.

摘要: A method of treating a tumor includes providing a tissue biopsy and treatment apparatus that includes an elongated delivery device that has a lumen and is maneuverable in tissue. A sensor array having a plurality of resilient members is deployable from the elongated delivery device. At least one of the plurality of resilient members is positionable in the elongated delivery device in a compacted state and deployable with curvature into tissue from the elongated delivery device in a deployed state. At least one of the plurality of resilient members includes at least one of a sensor, a tissue piercing distal end or a lumen. The sensor array has a geometric configuration adapted to volumetrically sample tissue at a tissue site to differentiate or identify tissue at the tissue site. At least one energy delivery device is coupled to one of the sensor array, at least one of the plurality of resilient members or the elongated delivery device. The apparatus is then introduced into a target tissue site. The sensor array is then utilized to distinguish a tissue type. The tissue type information derived from the sensor array is utilized to position the energy delivery device to ablate a tumor volume. Energy is then delivered from the energy delivery device to ablate or necrose at least a portion of the tumor volume. The sensor array is then utilized to determine an amount of tumor volume ablation.

摘要: A tissue surface treatment apparatus includes a housing having a proximal end, a distal end including a tissue contacting surface, an interior defined by the housing and a handpiece coupled to the housing. The contactinc surface has a plurality of apertures. An energy delivery device including an electrode is positionable in the housing interior. The electrode includes a tissue penetrating distal end in substantial alignment with an aperture. The electrode is configured to be advanced from the housing interior through the aperture and into a target tissue site to define an ablation volume at least partly bounded by a tissue surface. An advancement device is coupled to the energy delivery device and is at least partly positionable within at least one of the housing or the handpiece. The advancement device is configured to advance the electrode from the housing interior into the target tissue site and withdrawal the electrode into the housing interior. A cable is coupled to the housing or the energy delivery device and is configured to be coupled to an energy source.

The slidable sensing member is positionable within the introducer and electrically coupled to the energy delivery device. The sensing member is configured to measure a property of the energy delivery device or at least one electrode of the plurality of electrodes.
