摘要: A medical probe device comprises a catheter having a stylet guide housing with one or more stylet ports in a side wall thereof and a stylet guide for directing a flexible stylet outward through the stylet port and through intervening tissue at a preselected, adjustable angle to a target tissue. The total catheter assembly includes a stylet guide lumen communicating with the stylet port and a stylet positioned in said stylet guide lumen for longitudinal movement from the port through intervening tissue to a target tissue. The stylet can be an electrical conductor enclosed within a non-conductive layer, the electrical conductor being a radiofrequency electrode. Preferably, the non-conductive layer is a sleeve which is axially moveable on the electrical conductor to expose a selected portion of the electrical conductor surface in the target tissue. The stylet can also be a microwave antenna. The stylet can also be a hollow tube for delivering treatment fluid to the target tissue. It can also include a fiber optic cable for laser treatment. The catheter can include one or more inflatable balloons located adjacent to the stylet port for anchoring the catheter or dilation. Ultrasound transponders and temperature sensors can be attached to the probe end and/or stylet. The stylet guide can define a stylet path from an axial orientation in the catheter through a curved portion to a lateral orientation at the stylet port.

摘要: An infusion array ablation apparatus includes an elongated delivery device having a lumen and an infusion array positionable in the lumen. The infusion array includes an RF electrode and at least a first and a second infusion member. Each infusion member has a tissue piercing distal portion and an infusion lumen. At least one of the first or second infusion members is positionable in the elongated delivery device in a compacted state and deployable from the elongated delivery device with curvature in a deployed state. Also, at least one of the first or second infusion members exhibits a changing direction of travel when advanced from the elongated delivery device to a selected tissue site. At least one infusion port is coupled to one of the elongated delivery device, the infusion array, the first infusion member or the second infusion member.

摘要: A method of treating an intervertebral disc includes inserting an introducer into an intervertebral disc, inserting a catheter through the introducer and into the intervertebral disc, advancing a distal region of the catheter through a nucleus pulposus of the intervertebral disc to an annular wall of an annulus fibrosus, delivering energy to material of the intervertebral disc using an electrode disposed at the distal region of the catheter, and removing the material with the delivered energy, wherein removing material with the delivered energy reduces pressure in the intervertebral disc.

The heart chamber is partitioned so as to isolate a non-productive portion having a rupture or a region of an incipient rupture from a productive portion. The heart chamber is preferably partitioned with a reinforced membrane which has a pressure receiving surface that defines part of the productive portion of the heart chamber. The peripheral base of the reinforced membrane may have an eccentric configuration.

摘要: This invention is directed to a partitioning device for separating a patient's heart chamber into a productive portion and a non-productive portion. The device is particularly suitable for treating patients with congestive heart failure. The partitioning device has a reinforced, expandable membrane which separates the productive and non-productive portions of the heart chamber and a support or spacing member extending between the reinforced membrane and the wall of the patient's heart chamber. The support or spacing member has a non-traumatic distal end to engage the ventricular wall.

摘要: A tissue ablation apparatus comprises a first elongated delivery device including a lumen and an obturator with a tissue piercing distal end. The obturator is positionable in the lumen of the first elongated delivery device. A second elongated delivery device is positionable in the lumen of the first elongated delivery device. An energy delivery device is positionable in the second elongated delivery device. The energy delivery device includes at least a first and a second RF electrode each with a tissue piercing distal portion. The first and second RF electrodes are positionable in the second elongated delivery device in a compacted state and deployable from the second elongated delivery device with curvature in a deployed state. The first and second RF electrodes exhibit a changing direction of travel when advanced from the second elongated delivery device to a selected tissue site. At least one infusion port is coupled to one of the first elongated delivery device, the second elongated delivery device, the energy delivery device, the first RF electrode or the second RF electrode. An electrode advancement member is coupled to the first and second RF electrodes. The advancement member is configured to advance the RF electrode out of the elongated delivery device.

摘要: A method of creating a lesion in tissue with infusion includes providing an apparatus comprising a first elongated delivery device with a lumen, an obturator with a second elongated delivery device and an energy delivery device positional in a lumen of the first elongated delivery device. The energy device includes at least a first and a second RF electrode each with a tissue piercing distal portion, the first and second RF electrodes being deployable from the first elongated delivery device with curvature in a deployed state. The energy delivery device includes an infusion lumen and at least one infusion port. The obturator and second delivery device are introduced to a selected tissue site. The obturator is removed from a lumen of the second delivery device and the first delivery device and energy delivery device are introduced into the lumen of the second delivery device. The first and second RF electrodes are advanced from the first delivery device to at least partially surround a target tissue site. The target tissue site is at least partially infused with an infusion fluid. Energy is delivered from the energy delivery device to the target tissue site and cell necrosis is created at the target tissue site.

摘要: A device is described that may be positioned at a location in an intervertebral disc for diagnosis or treatment of the disc. Treatment may include, for example, applying energy or removing material, and may decrease intradiscal pressure. Radiofrequency energy may be applied. A percutaneous method of repairing a fissure in the annulus pulposus comprises placing an energy source adjacent to the fissure and providing sufficient energy to the fissure to raise the temperature to at least about 45-70° C. and for a sufficient time to cause the collagen to weld. An intervertebral fissure also can be treated by placing a catheter with a lumen adjacent to the fissure and injecting sealant into the fissure via the catheter, thereby sealing the fissure. An intervertebral fissure additionally can be treated by providing a catheter having a distal end, a proximal end, a longitudinal axis, and an intradiscal section at the catheter's distal end on which there is at least one functional element. The next step is applying a force longitudinally to the proximal of the catheter which is sufficient to advance the intradiscal section through the nucleus pulposus and around an inner wall of an annulus fibrosus, but which force is insufficient to puncture the annulus fibrosus. Next the functional element is positioned at a selected location of the disc by advancing or retracting the catheter and optionally twisting the proximal end of the catheter. Then the functional unit treats the annular fissure. Optionally, there is an additional step of adding a substance to seal the fissure. An externally guidable intervertebral disc apparatus also is disclosed.

摘要: A system for improving cardiac function is provided. A foldable and expandable frame having at least one anchoring formation is attached to an elongate manipulator and placed in a catheter tube while folded. The tube is inserted into a left ventricle of a heart where the frame is ejected from the tube and expands in the left ventricle. Movements of the elongate manipulator cause the anchor to penetrate the heart muscle and the elongate manipulator to release the frame. The installed frame minimizes the effects of an akinetic portion of the heart forming an aneurysmic bulge.
